The objectives are to study 1. The effectiveness of the telenursing self-management program on health behaviors of type 2 diabetes patients at the Nakhon Pathom Rajabhat University Urban Community Health Center and 2. The level of satisfaction with the telenursing self-management program of type 2 diabetes patients. The sample consists of 23 type 2 diabetes patients. The research instruments are health behavior questionnaires. The statistics used are percentage, mean, standard deviation, Wilcoxon signed rank test</p> <p>The research results found that</p> <p>1. The experimental group of type 2 diabetic patients who received the self-management program via telenursing had health behaviors and blood sugar levels after the experiment, which were significantly better than before the experiment (p&lt;.001). The mean scores of health behaviors and blood sugar levels after the experiment of the experimental group were significantly better than the control group (p&lt;.001).</p> <p>2. Type 2 diabetic patients in the experimental group who received the telenursing self-management program had a high level of overall satisfaction with the telenursing self-management program ( <img src="https://latex.codecogs.com/svg.image?\overline{X}" alt="equation" width="11" height="10">&nbsp;= 4.69). When considering each aspect separately, it was found that nursing had the highest level of satisfaction ( <img src="https://latex.codecogs.com/svg.image?\overline{X}" alt="equation" width="11" height="10">&nbsp;= 4.74), followed by knowledge about health care ( <img src="https://latex.codecogs.com/svg.image?\overline{X}" alt="equation" width="11" height="10">&nbsp;= 4.69), knowledge about treatment of type 2 diabetes and hypertension ( <img src="https://latex.codecogs.com/svg.image?\overline{X}" alt="equation" width="11" height="10">&nbsp;= 4.65), communication via LINE ( <img src="https://latex.codecogs.com/svg.image?\overline{X}" alt="equation" width="11" height="10">&nbsp;= 4.65).The telenursing self-management program encouraged patients to have good health behaviors. The study was conducted in three phases: situation analysis, guideline development, and outcome evaluation. The participants included 10 multidisciplinary healthcare team members and 42 newly diagnosed HCV patients. Research instruments consisted of the nursing guidelines for HCV patients, knowledge and self-care behavior questionnaires, satisfaction surveys, and structured interviews. Quantitative data were analyzed using descriptive statistics and paired t-tests, while qualitative data were analyzed through content analysis.</p> <p>The results revealed that.</p> <ol> <li class="show">Situation analysis identified key problems in HCV patient care, including the lack of a clear referral and follow-up system, multiple hospital visits before medication initiation, and insufficient self-care promotion, leading to high rates of missed appointments, medication non-adherence, and delayed treatment.</li> </ol> <p>2.The nursing guideline was developed, consisting of five components (1) a comprehensive initial service system, (2) an 8-week self-care promotion program, (3) a discharge planning guideline (DMETHOD KH<sup>2</sup>), (4) a seamless referral system, and (5) a follow-up system for post-treatment monitoring.</p> <ol start="3"> <li class="show">Outcome evaluation showed that patients’ knowledge and self-care behavior scores significantly improved (p &lt; 0.001). Appointment default rates decreased to 0%, medication non-adherence reduced to 2.38%, waiting times were shortened, service efficiency improved, and the treatment success rate reached 100%. The study was conducted with two female patients diagnosed with thyroid nodules who underwent surgery at Mahasarakham Hospital between July 2024 and January 2025. The research employed Gordon’s Functional Health Patterns for health assessment and Roy’s Adaptation Model as the theoretical framework to evaluate and compare outcomes including pain levels, postoperative complications, length of hospital stay, and patient satisfaction.</p> <p style="font-weight: 400;">Findings revealed that the patient who underwent TOETVA reported less postoperative pain, faster recovery, and higher satisfaction regarding cosmetic outcomes, as the procedure leaves no visible scar. However, some early postoperative issues such as intraoral discomfort and mild facial swelling were observed. In contrast, the patient who underwent open thyroidectomy experienced longer recovery and visible scarring but demonstrated stable clinical progress with no significant complications such as hoarseness or swallowing difficulty.</p> <p style="font-weight: 400;">The study’s findings contribute to the development of individualized nursing care plans tailored to specific surgical approaches. The service system was developed collaboratively by a multidisciplinary team based on the World Health Organization's guidelines: "6 Building Blocks of a Health System." The research period was December 2023 - March 2025. The system development was evaluated based on the outcomes of end-of-life care for 2,186 patients at the community-based hospital care center. Quantitative data were analyzed using descriptive statistics, including counts and percentages. Content analysis was used for qualitative data.</p> <p style="font-weight: 400;">The results of the study revealed that the community-based hospital care center service system for end-of-life care was developed. Maha Sarakham Province consists of 1) organizing a system to support the provision of care services by driving operations through provincial and district level committees, managing life care centers according to standards, organizing a system to support resources and medical equipment; 2) managing an efficient care system with a Project Manager and Case Manager: CM (Palliative Care Nurse) to coordinate, develop and link the entire system. Develop care guidelines, CPG manuals, media, LINE groups, a consulting system, a referral system, and an information system, along with integrating technology into care. 3) Develop human resources based on the competencies of each profession, both within hospitals and within professional nurses in subdistrict health promotion hospitals. This includes developing village health volunteers (VHVs) and family caregivers to enhance their capacity to provide continuous care at home. Evaluation revealed that Maha Sarakham Province has established and opened community hospital lifecare centers that meet the standard criteria in all 12 districts 100%,100% of the multidisciplinary team's competency was developed, with 96.75% of families participating in family meetings. High-quality advance care planning was provided for palliative care patients 89.20% and for patients with the good death 93.45%.</p> <p><span style="font-weight: 400;">Conclusion: The development of a community-based hospital lifecare center service system for end-of-life care, developed through participation and using the Six Building Blocks of a Health System as a guideline, has resulted in efficient service delivery that addresses the needs of end-of-life care and adequately allocates resources. The incidence of type 1 diabetes in children and adolescents is also rising, with most cases diagnosed in children under 15 years old. This disease impacts children and adolescents physically, mentally, emotionally, and socially. The inability to maintain blood sugar levels within the appropriate range leads to both acute and chronic effects, including complications in other organ systems. Additionally, it places a burden on the families of affected children due to the need for continuous care, as well as increased medical and childcare costs. Therefore, pediatric and adolescent patients should be provided with knowledge and support for self-care to promote appropriate behavioral modification, including dietary control, insulin management, exercise, and stress management, all of which can help delay the onset of long-term complications. </p> <p style="font-weight: 400;">This article presents an approach for promoting self-management through a case study of adolescent patients with type 1 diabetes, utilizing self-management concepts and Creer's framework. The study encouraged patient involvement in self-care activities such as goal setting, data collection, data analysis and evaluation, decision-making, implementation, and self-reflection. The study was conducted from December 2024 to June 2025 using the Evidence-Based Practice Model by Soukup (2000), which consists of four phases: (1) identifying clinical problems, (2) searching for empirical evidence, (3) developing and piloting the guidelines, and (4) implementing the guidelines in practice.The sample consisted of 30 registered nurses, 30 preterm infants, and 30 primary caregivers selected through purposive sampling. Data collection instruments included focus group discussions, a competency assessment form, a satisfaction questionnaire, and a caregiver knowledge test. Research tools included the developed nursing practice guidelines and the Respiratory Distress Score (RD Score) assessment tool. Data were analyzed using descriptive statistics, percentage, and Paired Sample T-Test.</p> <p style="font-weight: 400;">The findings showed that the developed nursing practice guidelines encompassed four key components 1) guidelines for preterm infant care 2) nurse competencies 3) caregiver knowledge and skills, and (4) clinical supervision. After implementation, the accurate use of the RD Score by nurses increased to 90%, and the incidence of unanticipated clinical deterioration and unplanned admissions to the neonatal intensive care unit decreased by 6.66. Nurse competencies in all domains significantly improved (P &lt; .05), as did caregiver knowledge (P &lt; .001).
