บทคัดย่อ

ประเทศไทยสามารถบรรลุเป้าหมายในการพัฒนาระบบหลักประกันสุขภาพถ้วนหน้า (universal health coverage: UHC) แต่ยังมีความท้าทายที่สำคัญด้านความไม่เสมอภาคของผู้ใช้บริการระบบสาธารณสุข ได้แก่ ความคุ้มครองภาระค่าใช้จ่ายที่ไม่เท่าเทียมกันและความเหลื่อมล้ำในคุณภาพของการรักษาพยาบาล ดังนั้น งานวิจัยนี้จึงได้ศึกษาความเหลื่อมล้ำในลูกบาศก์หลักประกันสุขภาพถ้วนหน้า ซึ่งมีมิติครอบคลุม 3 ด้าน ได้แก่ ด้านประชากร ด้านค่าใช้จ่ายการใช้บริการ และด้านบริการสุขภาพ เพื่อประเมินสถานการณ์ความเหลื่อมล้ำในระบบหลักประกันสุขภาพถ้วนหน้าสำหรับประชากรแต่ละกลุ่ม จำแนกกลุ่มประชากรตามระบบหลักประกันสุขภาพ 3 กองทุนหลัก ระดับเศรษฐฐานะครัวเรือน เพศ กลุ่มอายุ ภูมิภาค และในเขตหรือนอกเขตเทศบาล โดยคำนวณจากค่าเฉลี่ยตัวแปรที่เกี่ยวข้องของข้อมูลสำรวจภาวะเศรษฐกิจและสังคมของครัวเรือน พ.ศ. 2563 ผลการศึกษาพบว่า ความเหลื่อมล้ำที่สูงที่สุด คือ ด้านบริการสุขภาพ หรือ คุณภาพ ซึ่งเกิดจากความเหลื่อมล้ำระหว่างแต่ละกองทุนของระบบหลักประกันสุขภาพ ดังนั้น ข้อค้นพบจึงเน้นย้ำความสำคัญของการเร่งขับเคลื่อนแผนปฏิรูปประเทศด้านสาธารณสุข เพื่อแก้ไขปัญหาความไม่เป็นธรรมด้านค่าใช้จ่ายเฉลี่ยต่อหัวและคุณภาพการให้บริการ โดยการบูรณาการ 3 กองทุนหลักของระบบหลักประกันสุขภาพของรัฐ
