ต้นทุนการดูแลผู้ป่วยมะเร็งแบบประคับประคองของโรงพยาบาลชุมชนในจังหวัดขอนแก่น

บทคัดย่อ

การดูแลแบบประคับประคองเป็นขั้นตอนสำคัญของการดูแลผู้ป่วยมะเร็งโดยเฉพาะเมื่อเข้าสู่ระยะท้าย องค์การอนามัยโลกได้กล่าวไว้ว่าการดูแลแบบประคับประคองนั้นมีต้นทุนต่ำ แต่การศึกษาต้นทุนของบริการประเภทนี้ในประเทศไทยยังมีจำกัด การศึกษาครั้งนี้จึงมีวัตถุประสงค์เพื่อศึกษาต้นทุนในการดูแลผู้ป่วยมะเร็งแบบประคับประคองของโรงพยาบาลชุมชนแห่งหนึ่งในจังหวัดขอนแก่น วิเคราะห์ต้นทุนในสองมุมมอง คือ ผู้ให้บริการและผู้รับบริการ เก็บข้อมูลต้นทุนจากผู้ป่วย 42 คน บันทึกต้นทุนที่เกิดขึ้นในช่วง 1 มกราคม – 31 ธันวาคม 2561 ต้นทุนในมุมมองผู้ให้บริการ ได้แก่ ต้นทุนค่าแรงและต้นทุนวัสดุของบริการ 3 แผนก ได้แก่ ผู้ป่วยนอก ผู้ป่วยในและการเยี่ยมบ้าน ต้นทุนในมุมมองผู้รับบริการ ได้แก่ ต้นทุนทางตรงด้านการแพทย์ ต้นทุนทางตรงที่ไม่ใช่ด้านการแพทย์และต้นทุนค่าเสียโอกาส ผลการศึกษาพบว่า ต้นทุนเฉลี่ยในมุมมองผู้ให้บริการเท่ากับ 16,525 ± 14,979 บาทต่อคน เกิดจากแผนกผู้ป่วยนอก 4,309 ± 7,103 บาทต่อคน แผนกผู้ป่วยใน 10,915 ± 11,704 บาทต่อคน และการเยี่ยมบ้าน 1,301 ± 1,977 บาทต่อคน ต้นทุนในมุมมองนี้เกิดจากต้นทุนวัสดุการแพทย์เป็นหลัก (ร้อยละ 68) ต้นทุนเฉลี่ยในมุมมองผู้รับบริการเท่ากับ 38,707 ± 32,943 บาทต่อคน เกิดจากการต้องมารับบริการที่แผนกผู้ป่วยนอก 5,262 ± 5,571 บาทต่อคน แผนกผู้ป่วยใน 6,231 ± 7,355 บาทต่อคน และต้นทุนจากการดูแลผู้ป่วยที่บ้าน 20,806 ± 20,454 บาทต่อคน และต้นทุนจากการแสวงหาการรักษาอื่น 6,408 ± 17,336 บาทต่อคน ต้นทุนในมุมมองนี้เกิดจากค่าเสียโอกาสมากที่สุด (ร้อยละ 69) ผลการศึกษาครั้งนี้น่าจะเป็นประโยชน์ต่อกระทรวงสาธารณสุขและสำนักงานหลักประกันสุขภาพแห่งชาติในการทบทวนการจัดสรรงบประมาณให้กับบริการประเภทนี้
