Factors Correlated with Survival of Road Injuries Patients of Tertiary Care Hospital in Nakhon Si Thammarat Province

Abstract
Assessing the severity of trauma patients using the Glasgow Coma Scale (GCS) alone may have limitations in identifying crucial physiological changes. This study aimed to investigate factors associated with survival and to develop a scoring system for predicting the survival of road traffic accident victims. Conducted on 435 patients admitted to a tertiary hospital in Nakhon Si Thammarat Province between January 2023 and December 2024, the researchers developed an integer-based scoring system (0–12 points) derived from logistic regression coefficients and evaluated the performance of the "Practical PS Score" in predicting the survival of these patients.
The results revealed that factors significantly associated with survival (p-value < 0.05) included age group and level of consciousness. Specifically, the elderly group (≥ 60 years) and patients with severe impairment of consciousness (GCS ≤ 8) had a significantly lower probability of survival (aOR = 0.33 and 0.26, respectively) compared to the working-age group. Furthermore, the newly developed Practical PS Score demonstrated higher accuracy in predicting survival than using the GCS alone. Establishing a cutoff score at ≤ 11 yielded a sensitivity of 88.4% and a negative predictive value (NPV) of 92.1%.
Therefore, the Practical PS Score is an effective and user friendly tool suitable for screening high-risk patients. It helps reduce under-triage rates and effectively supports clinical decision-making.
