บทคัดย่อ

ในปี พ.ศ. 2566 ประเทศไทยมีความร่วมมือเพื่อยกระดับมาตรฐานโรงพยาบาล/สถานพยาบาลปลอดบุหรี่ส่งเสริมสุขภาพไปสู่มาตรฐานเฉพาะโรค/เฉพาะระบบกลุ่มบำบัดนิโคติน การศึกษาเปรียบเทียบความแตกต่างของข้อกำหนดมาตรฐานทั้ง 2 นี้ จึงเป็นประโยชน์ต่อสถานพยาบาลที่มีแผนดำเนินการขอรับรองมาตรฐานเฉพาะโรค/เฉพาะระบบในกลุ่มภาวะติดนิโคติน ในการศึกษานี้มีการเปรียบเทียบข้อแตกต่างมาตรฐานทั้ง 2 เป็น 3 กรณีคือ 1) เอกสารในการขอรับรอง 2) จำนวนและรายละเอียดของข้อกำหนด และ 3) เกณฑ์การพิจารณา ผลการศึกษาพบว่า 1) เอกสารในการขอรับรองมีความแตกต่างกันทั้งจำนวนและรายการ โดยมาตรฐานเฉพาะโรค/เฉพาะระบบจะมีรายการเอกสารมากกว่า 2) ข้อกำหนดของสองมาตรฐานมีจำนวนเท่ากัน แต่ต่างกันในรายละเอียดของเนื้อหา โดยมาตรฐานโรงพยาบาล/สถานพยาบาลปลอดบุหรี่มีความจำเพาะในกระบวนการและผลลัพธ์ของการเลิกบุหรี่ ส่วนข้อกำหนดของมาตรฐานเฉพาะโรค/เฉพาะระบบเน้นกระบวนการดูแลผู้ป่วย และระบบงานสำคัญ โดยเน้นการทำงานร่วมกันของทีมสหสาขาวิชาชีพ 3) เกณฑ์การพิจารณาอ้างอิงมาตรฐานตาม scoring guideline 1-5 คะแนน เหมือนกันทั้ง 2 มาตรฐาน โดยมาตรฐานโรงพยาบาล/สถานพยาบาลปลอดบุหรี่แยกเกณฑ์มาตรฐานข้อที่ 1-7 กับมาตรฐานข้อที่ 8 และยังมีการคำนวณคะแนนด้วยค่าถ่วงน้ำหนักของแต่ละข้อมาตรฐาน ไม่กำหนดคะแนนขั้นต่ำ ในขณะที่มาตรฐานเฉพาะโรค/เฉพาะระบบกำหนดให้ตอนที่ III กระบวนการดูแลผู้ป่วย ควรได้คะแนนมากกว่า 3.0 โดย สถานพยาบาลควรเริ่มจาก 1) จัดตั้งคณะทำงานดำเนินการที่ประกอบด้วยทีมสหสาขาวิชาชีพ 2) จัดทำแผนปฏิบัติในการขอรับรอง 3) กำหนดตัวชี้วัด พร้อมกำหนดค่าเป้าหมาย 4) ส่งเสริมการพัฒนางานด้วยผลงานวิชาการ ทั้งนี้ สำหรับสถานพยาบาลที่เริ่มต้นใหม่อาจจะใช้ระยะเวลาในการดำเนินการอย่างน้อย 3 ปี เพื่อให้ได้เห็นผลลัพธ์กระบวนการดูแลและกระบวนการพัฒนาของสถานพยาบาล
