บทคัดย่อ

งานวิจัยนี้ใช้วิธีการวิจัยเชิงคุณภาพโดยการทบทวนเอกสาร มีวัตถุประสงค์เพื่อศึกษาความก้าวหน้าของการถ่ายโอนสถานบริการสุขภาพให้แก่องค์กรปกครองส่วนท้องถิ่น ตามแผนปฏิบัติการกำหนดขั้นตอนการกระจายอำนาจให้แก่องค์กรปกครองส่วนท้องถิ่นฉบับที่ 1 และฉบับที่ 2 รวมถึงหลักเกณฑ์และขั้นตอนการถ่ายโอนภารกิจสถานีอนามัยเฉลิมพระเกียรติ 60 พรรษา นวมินทราชินี และโรงพยาบาลส่งเสริมสุขภาพตำบลให้แก่องค์การบริหารส่วนจังหวัด พ.ศ. 2564 ผลการศึกษา พบว่า หลักเกณฑ์และเงื่อนไขในการถ่ายโอนภารกิจสถานบริการสุขภาพให้แก่องค์กรปกครองส่วนท้องถิ่นของประเทศไทย จากอดีตถึงปัจจุบันมีการปรับปรุงให้มีความชัดเจนและยืดหยุ่นมากขึ้นมาอย่างต่อเนื่อง โดยเฉพาะเกณฑ์การประเมินความพร้อมขององค์กรปกครองส่วนท้องถิ่นที่มีการแบ่งระดับเป็นขั้น เพื่อให้ทุกองค์กรสามารถรับการถ่ายโอนภารกิจได้ตามศักยภาพของตนเอง อย่างไรก็ดี ประเด็นที่ยังคงเป็นความท้าทายมาโดยตลอดก็คือกฎหมายและระเบียบรองรับการดำเนินงาน เช่น การควบคุมการประกอบวิชาชีพเวชกรรมและอื่นๆ การถ่ายโอนบุคลากรทุกประเภท การบริหารจัดการเงินกองทุนหลักประกันสุขภาพแห่งชาติระหว่างหน่วยบริการประจำและโรงพยาบาลส่งเสริมสุขภาพตำบลที่ถ่ายโอน แต่ยังพบปัญหา อุปสรรค คือขาดความต่อเนื่องในการจัดทำแผนการกระจายอำนาจและการจัดทำแผนปฏิบัติการฯ ข้อจำกัดด้านงบประมาณ ข้อจำกัดด้านกฎหมาย ส่วนปัญหาด้านการถ่ายโอนบุคลากร พบปัญหาความไม่ชัดเจนในการแบ่งหน้าที่ระหว่างหน่วยงานที่เกี่ยวข้อง การศึกษานี้มีข้อเสนอเชิงนโยบาย คือ 1) ควรพิจารณาทบทวน ปรับปรุงและแก้ไขกฎระเบียบของกระทรวงมหาดไทยที่เกี่ยวกับการดําเนินงานของหน่วยบริการสุขภาพที่ถูกถ่ายโอน เพื่อให้การบริหารจัดการและการให้บริการมีประสิทธิภาพ 2) ควรพิจารณาแก้ไขปรับปรุงกฎระเบียบต่างๆ ในการเพิ่มกรอบอัตรากําลังบุคลากร เพื่อให้สถานบริการสุขภาพที่ถ่ายโอนให้แก่องค์กรปกครองส่วนท้องถิ่นมีความยืดหยุ่นและคล่องตัวตามบริบทของตน ตามปัญหาและข้อจํากัดในแต่ละพื้นที่ภายใต้กรอบวงเงินงบประมาณด้านบุคลากร และ 3) ควรมีการจัดสรรงบประมาณตามภาระงานที่สถานบริการสุขภาพที่ถ่ายโอนไปในแต่ละพื้นที่ต้องรับผิดชอบ เพื่อสถานบริการเหล่านั้นจะได้มีความมั่นคงด้านรายได้ในระดับหนึ่ง รวมทั้งสามารถวางแผนการจัดการทรัพยากรและรายได้ของตนเอง เพื่อจัดบริการสาธารณะให้แก่ประชาชนในพื้นที่ได้อย่างมีประสิทธิภาพ คุ้มค่าและ เกิดประสิทธิผล
