บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาเปรียบเทียบต้นทุนต่อหน่วยของบริการในโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ที่ถ่ายโอนไปเทศบาลเมือง และเทศบาลตำบล และ รพ.สต. ที่ยังสังกัดกระทรวงสาธารณสุข ใช้วิธีวิเคราะห์ต้นทุนต่อหน่วยแบบมาตรฐาน โดยคัดเลือกพื้นที่จำนวน 2 จังหวัด แบบเฉพาะเจาะจง (purposive sampling) ได้แก่ เชียงราย และลำปาง และเก็บข้อมูลในส่วนของต้นทุนค่าแรง ต้นทุนค่าวัสดุ และต้นทุนค่าลงทุน (ครุภัณฑ์) ซึ่งเป็นข้อมูลย้อนหลัง ระหว่างเดือนตุลาคม พ.ศ. 2564 - เดือนกันยายน พ.ศ. 2565 ข้อมูลข้างต้นได้จากการสัมภาษณ์ผู้ปฏิบัติงาน และข้อมูลที่มีบันทึกไว้ใน รพ.สต. จากนั้น จึงเก็บรวบรวมข้อมูลลงในแบบบันทึกข้อมูลเพื่อวิเคราะห์ต้นทุนต่อหน่วยของบริการใน รพ.สต. ผลการวิเคราะห์ต้นทุนรวมและผลการวิเคราะห์ต้นทุนต่อหน่วยของบริการ พบว่า ศูนย์บริการสาธารณสุข (ศบส.) ที่ถ่ายโอนไปเทศบาลเมือง และเทศบาลตำบลเพียงหนึ่งปี มีสัดส่วนต้นทุนค่าแรงจากต้นทุนทั้งหมดน้อยกว่า รพ.สต. ที่ยังสังกัดกระทรวงสาธารณสุข รวมถึงมีต้นทุนต่อหน่วยในทุกบริการน้อยกว่า รพ.สต. ที่ยังสังกัดกระทรวงสาธารณสุข สาเหตุหลัก คือ ต้นทุนค่าแรงของบุคลากรลดลง ซึ่งเป็นผลมาจากบุคลากรเดิมไม่ได้สมัครใจถ่ายโอนทั้งหมด ขณะที่ ศบส. ที่ถ่ายโอนไปยังเทศบาลตำบลตั้งแต่ช่วงยุคแรกของการถ่ายโอน พบว่า ต้นทุนต่อหน่วยของบริการใกล้เคียงกับ รพ.สต. ที่ยังสังกัดกระทรวงสาธารณสุข ต้นทุนค่าแรงที่ลดลงสะท้อนให้เห็นว่า จำนวนบุคลากรหลังการถ่ายโอนลดลง ดังนั้น จึงควรมีการเตรียมความพร้อมของบุคลากรก่อนการถ่ายโอน อย่างไรก็ตาม การศึกษานี้มีข้อจำกัดที่การวิเคราะห์ข้อมูลต้นทุนต่อหน่วยของบริการใน รพ.สต. เป็นข้อมูลในช่วงปีที่ยังมีการให้บริการในกิจกรรมที่เกี่ยวข้องกับโควิด-19 และข้อมูลบางส่วนที่ใช้ในการวิเคราะห์ต้นทุนต่อหน่วยเป็นข้อมูลจากการศึกษาในอดีต
