บทคัดย่อ

การประเมินสถานการณ์การใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชนทั่วประเทศยังไม่เคยถูกรายงาน งานวิจัยนี้จึงมีวัตถุประสงค์เพื่อศึกษาสถานการณ์การดำเนินงานด้านการใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชน และวิเคราะห์ปัจจัยที่มีผลต่อการดำเนินงาน โดยวิเคราะห์ข้อมูลจากแบบสอบถาม ตั้งแต่ปี พ.ศ. 2564–2567 และใช้สถิติเชิงพรรณนา แบบสอบถามแบ่งเป็น 3 ส่วน คือ 1) ข้อมูลทั่วไปของโรงพยาบาล 2) การดำเนินงานด้านการใช้ยาอย่างสมเหตุผล ตามแนวทางของโรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผล (แนวทาง PLEASE: pharmacy and therapeutic committee, labeling, essential tools, awareness, special populations, ethics in prescription) และ 3) คำถามปลายเปิดเฉพาะปี พ.ศ. 2567 ขอความเห็นเกี่ยวกับปัจจัยที่มีผลต่อการดำเนินงานด้านการใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชน โรงพยาบาลเอกชนให้ความร่วมมือในการตอบแบบสอบถามเพิ่มขึ้นทุกปีจากร้อยละ 27.8 เป็น 38.1, 39.1 และ 49.3 ตามลำดับ ผลการดำเนินงานด้านการใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชนตามแนวทางของโรงพยาบาลส่งเสริมการใช้ยาอย่างสมเหตุผล (แนวทาง PLEASE) ดีขึ้นอย่างต่อเนื่อง โดยคิดเป็นร้อยละ 65.7, 68.8, 71.6 และ 74.2 ตามลำดับ หัวข้อที่โรงพยาบาลสามารถดำเนินการได้มากที่สุด คือ การจัดทำฉลากยา ฉลากยาเสริมและข้อมูลยาสู่ประชาชน (ร้อยละ 83.0-93.7) ทั้งนี้อาจเนื่องจากการจัดทำฉลากยาถูกบังคับด้วยกฎหมายตั้งแต่ปี พ.ศ. 2565 ในขณะที่การจัดทำหรือจัดหาเครื่องมือจำเป็นที่ช่วยให้เกิดการสั่งใช้ยาอย่างสมเหตุผล (ร้อยละ 57.9-68.4) และการสร้างความตระหนักรู้ของบุคลากรทางการแพทย์และผู้รับบริการต่อการใช้ยาอย่างสมเหตุผล (ร้อยละ 65.1-70.2) เป็นสองหัวข้อที่โรงพยาบาลเอกชนดำเนินงานได้น้อยที่สุด ความร่วมมือและการทำงานเป็นทีมของบุคลากรทางการแพทย์ นโยบาย บทบาทและการสนับสนุนของผู้บริหารโรงพยาบาล เทคโนโลยีที่สนับสนุนงานบริการในโรงพยาบาล ทัศนคติของผู้รับบริการ ตลอดจนนโยบายและการควบคุมกำกับจากหน่วยงานส่วนกลางเป็นปัจจัยสำคัญที่มีผลต่อการดำเนินงานด้านการใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชน ผลที่ได้จากงานวิจัยนี้สามารถใช้เป็นแนวทางในการพัฒนาระบบบริการสุขภาพด้านการใช้ยาอย่างสมเหตุผลของโรงพยาบาลเอกชนทั่วประเทศ เพื่อยกระดับโรงพยาบาลเอกชนให้มีมาตรฐานด้านระบบยา ส่งผลให้ประชาชนผู้รับบริการมีความปลอดภัยจากการใช้ยาและลดค่าใช้จ่ายด้านยาโดยไม่จำเป็น
