การประเมินเชิงพัฒนาการเข้าถึงและคุณภาพบริการดูแลระยะกลางการปฏิรูประบบบริการสุขภาพปฐมภูมิในกรุงเทพมหานคร

การดูแลระยะกลาง หรือ intermediate care (IMC) คือการดูแลผู้ป่วยที่ผ่านพ้นระยะเฉียบพลัน (acute phase) และมีอาการคงที่แล้ว แต่ยังคงมีความบกพร่องทางร่างกายบางส่วนที่จำกัดการทำกิจวัตรประจำวันและการมีส่วนร่วมในสังคม เช่น ผู้ป่วยที่เป็นโรคหลอดเลือดสมองที่ต้องการการฟื้นฟูสภาพร่างกาย การศึกษานี้ติดตามผู้ป่วยโรคหลอดเลือดสมองที่ออกจากโรงพยาบาลในระยะ 6 เดือนแรก ในการได้รับบริการระยะกลาง (IMC) จำนวน 139 ราย จาก 5 โรงพยาบาล เป็นเพศหญิง 65 ราย (ร้อยละ 46.8) และเพศชาย 74 ราย (ร้อยละ 53.2) อายุเฉลี่ย 70.6 ปี ส่วนใหญ่เป็นโรคหลอดเลือดสมองอุดตัน (ischemic stroke) 66 ราย (ร้อยละ 77.7) และหลอดเลือดสมองแตก (hemorrhagic stroke) 19 ราย (ร้อยละ 22.4) รูปแบบการรับบริการ IMC ได้แก่ IMC ward 31 ราย (ร้อยละ 22.3) IMC outpatient 27 ราย (ร้อยละ 19.4) IMC home rehabilitation 21 ราย (ร้อยละ 15.1) และ self-home rehabilitation 45 ราย (ร้อยละ 32.4) พบว่า ผลลัพธ์ทั้งด้านการฟื้นตัวด้วย Barthel index (BI) ด้านคุณภาพชีวิตด้วยแบบสอบถาม EQ-5D-5L และสุขภาวะด้วยแบบสอบถาม WHO5 (World Health Organization-five well-being index) ทุกกลุ่มมีพัฒนาการของ BI, EQ-5D-5L, และ WHO5 เพิ่มขึ้นในทุกรูปแบบการบริการ IMC สรุปคือการพัฒนาและขยายบริการ IMC เป็นแนวทางสำคัญในการยกระดับคุณภาพชีวิตและการฟื้นตัวของผู้ป่วยโรคหลอดเลือดสมองที่เป็นปัญหาสำคัญของการขับเคลื่อนระบบบริการสุขภาพปฐมภูมิของกรุงเทพมหานครให้มีประสิทธิภาพ โดย IMC home rehabilitation เป็นทางเลือกที่ให้ผลลัพธ์ใกล้เคียงกับ IMC ward ซึ่งทำให้ผู้ป่วยมี BI ดีขึ้นอย่างมีนัยสำคัญทางสถิติหลังจากผ่านไป 14 สัปดาห์
