บทคัดย่อ

ภูมิหลังและเหตุผล: จังหวัดปราจีนบุรีได้ถ่ายโอนภารกิจโรงพยาบาลส่งเสริมสุขภาพตำบล (รพ.สต.) ให้แก่องค์การบริหารส่วนจังหวัด (อบจ.) อย่างสมบูรณ์ พร้อมกับการปรับใช้เกณฑ์มาตรฐานบริการสุขภาพปฐมภูมิใหม่ การรักษาและพัฒนาคุณภาพการให้บริการสุขภาพจึงเป็นสิ่งสำคัญ เพื่อให้ประชาชนเข้าถึงบริการสุขภาพที่มีคุณภาพและมาตรฐานอย่างต่อเนื่อง การวิจัยนี้มีวัตถุประสงค์เพื่อ (1) ศึกษาผลการพัฒนาคุณภาพการจัดบริการสุขภาพ (2) เปรียบเทียบผลการพัฒนาคุณภาพการจัดบริการสุขภาพ ปีงบประมาณ พ.ศ. 2566 และพ.ศ. 2567 และ (3) ศึกษาปัจจัยแห่งความสำเร็จและปัญหาอุปสรรคในการดำเนินการพัฒนาคุณภาพการจัดบริการสุขภาพของโรงพยาบาลส่งเสริมสุขภาพตำบลหลังการถ่ายโอนภารกิจให้องค์การบริหารส่วนจังหวัดปราจีนบุรี

ระเบียบวิธีศึกษา: เป็นการวิจัยผสมวิธี (mixed methods research) ดำเนินการวิจัยเชิงปริมาณโดยการเปรียบเทียบข้อมูลทุติยภูมิจากคะแนนประเมินคุณภาพบริการสุขภาพปฐมภูมิ 8 ด้าน ของ รพ.สต. 94 แห่งด้วยสถิติเชิงพรรณนาและสถิติทดสอบ Wilcoxon signed-rank และดำเนินการวิจัยเชิงคุณภาพ โดยสัมภาษณ์เชิงลึก คัดเลือกกลุ่มตัวอย่างแบบเจาะจง คือ ผู้อำนวยการโรงพยาบาลส่งเสริมสุขภาพตำบลที่ขึ้นทะเบียนเป็นหน่วยบริการปฐมภูมิและเครือข่ายหน่วยบริการปฐมภูมิ 31 คน วิเคราะห์ข้อมูลด้วยการวิเคราะห์เนื้อหา ศึกษาระหว่างเดือนกุมภาพันธ์ 2567 ถึงเดือนกรกฎาคม 2568

ผลการศึกษา: พบว่า (1) ผลการพัฒนาคุณภาพการจัดบริการสุขภาพของโรงพยาบาลส่งเสริมสุขภาพตำบลภายหลังการถ่ายโอน ส่วนใหญ่สะท้อนว่าคุณภาพบริการโดยรวมยังคงรักษามาตรฐานได้ดีขึ้นหรือเท่าเดิมในเกือบทุกด้าน การบริหารจัดการมีความคล่องตัวมากขึ้น มีการจัดบริการที่สอดคล้องกับบริบทพื้นที่ และได้รับการสนับสนุนจากภาคีเครือข่าย ส่งผลให้หน่วยบริการสามารถตอบสนองความต้องการของประชาชนได้ดีขึ้น (2) ผลการเปรียบเทียบระหว่างปีงบประมาณ พ.ศ. 2566 และ พ.ศ. 2567 พบว่า โรงพยาบาลส่งเสริมสุขภาพตำบลมีคุณภาพการจัดบริการสุขภาพเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p < 0.05) โดยเฉพาะด้านการบริหารจัดการ การจัดการบุคลากร ระบบสารสนเทศและการจัดบริการเภสัชกรรม เนื่องจากนโยบายและเป้าหมายจาก อบจ. มีความชัดเจน ทำให้ รพ.สต. มีทิศทางการดำเนินงานอย่างเป็นรูปธรรมและมีประสิทธิภาพ และ (3) ปัจจัยความสำเร็จสำคัญ ได้แก่ การสนับสนุนเชิงนโยบายและทรัพยากรจากองค์การบริหารส่วนจังหวัด การบริหารจัดการที่มีประสิทธิภาพ และความร่วมมือของบุคลากรและชุมชน ปัญหาอุปสรรค ได้แก่ ภาระงานเอกสารที่เพิ่มขึ้น บุคลากรไม่เพียงพอ ระบบสารสนเทศไม่ครอบคลุม และเกณฑ์ประเมินที่มีความซับซ้อนบางด้าน
