บทคัดย่อ

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนรูปแบบบริการสุขภาพ (health service models) ผู้ป่วยโควิด-19 ที่โรงพยาบาลจัดไว้สำหรับรองรับการรักษาตามระดับอาการของผู้ป่วย โดยวิเคราะห์ต้นทุนในมุมมองผู้ให้บริการ ใช้การวิเคราะห์ต้นทุนบริการแบบมาตรฐาน (standard costing method) และวิเคราะห์ต้นทุนรายผู้ป่วยด้วยวิธีต้นทุนจุลภาคหรือจากล่างขึ้นบน (micro-costing or bottom-up approach) ใช้ข้อมูลจากโรงพยาบาลศูนย์ 3 แห่ง และโรงพยาบาลทั่วไป 2 แห่ง รวม 5 แห่ง ศึกษาต้นทุนปีงบประมาณ 2564 (ต.ค. 2563 - ก.ย. 2564) โดยคัดเลือกข้อมูลเฉพาะผู้ป่วยในที่เป็นโควิด-19 จากรหัสโรคที่โรงพยาบาลบันทึกไว้ รูปแบบบริการผู้ป่วยในโควิด-19 ของการศึกษานี้มี 7 รูปแบบ ได้แก่ 1) หอผู้ป่วยโควิด-19 (cohort ward) 2) หอผู้ป่วยวิกฤตโควิด-19 (intensive care unit-cohort ward 3) หอผู้ป่วยเฉพาะกิจ (hospitel) 4) โรงพยาบาลสนาม (field hospital) 5) โรงพยาบาลสนามในเรือนจำ (field hospital for prisoners) 6) กักตัวที่บ้าน (home isolation) และ 7) กักตัวในชุมชน (community isolation) ผลการศึกษา ต้นทุนการรักษาผู้ป่วยในโควิด-19 เฉลี่ย 31,342 บาทต่อราย วันนอนเฉลี่ย 10.9 วันต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยโควิด-19 62,033 บาทต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยวิกฤตโควิด-19 187,257 บาทต่อราย ต้นทุนผู้ป่วยที่หอผู้ป่วยเฉพาะกิจ 18,125 บาทต่อราย ต้นทุนผู้ป่วยที่โรงพยาบาลสนาม 19,589 บาทต่อราย ต้นทุนผู้ป่วยที่โรงพยาบาลสนามในเรือนจำ 24,318 บาทต่อราย ต้นทุนผู้ป่วยที่กักตัวที่บ้าน 6,039 บาทต่อราย ต้นทุนผู้ป่วยที่กักตัวในชุมชน 4,532 บาทต่อราย ต้นทุนของผู้ป่วยในโควิด-19 ที่มีการเปลี่ยนรูปแบบบริการในระหว่างการรักษาจากนอกโรงพยาบาลย้ายเข้ารักษาภายในโรงพยาบาล มีต้นทุนรวมเฉลี่ย 91,568 บาทต่อราย วันนอนรวมเฉลี่ย 11.0 วัน และต้นทุนผู้ป่วยที่เปลี่ยนรูปแบบบริการอยู่ภายในโรงพยาบาล (cohort ward ไป ICU-cohort ward) ต้นทุนรวมเฉลี่ย 149,844 บาทต่อราย วันนอนรวมเฉลี่ย 12.1 วัน ข้อเสนอแนะ งานวิจัยนี้ สามารถสะท้อนให้เห็นถึงต้นทุนการใช้ทรัพยากรในแต่ละรูปแบบบริการของผู้ป่วยในโควิด-19 ซึ่งมีความแตกต่างกัน หากศึกษาเพิ่มเติมเกี่ยวกับจำนวนงบประมาณ ที่กองทุนประกันสุขภาพจ่ายชดเชยค่าบริการให้กับโรงพยาบาล เปรียบเทียบกับต้นทุนที่แท้จริง จะสามารถสะท้อนความพอเพียงของงบประมาณ และวิธีการจ่ายเงินของกองทุนว่าเหมาะสมสอดคล้องกับการใช้ทรัพยากรของโรงพยาบาลเพียงใด
