บทคัดย่อ

การพัฒนาระบบสารสนเทศแบบบูรณาการ ประกอบด้วย การลงทะเบียนและการสร้างแหล่งเก็บข้อมูลเพื่อใช้ประโยชน์ร่วมกัน ระบบการใช้ประโยชน์สารสนเทศเป็นประเด็นหนึ่งของกรอบแนวความคิดการบริการแบบมุ่งเน้นคุณค่า (value-based healthcare) โดยส่วนหนึ่งคือโครงสร้างตัวชี้วัดที่เหมาะสมและรองรับกับฐานข้อมูลผู้ป่วย ประกอบกับความครบถ้วนและความถูกต้องของฐานข้อมูลนั้นเป็นสิ่งที่สำคัญต่อการนำไปใช้ประโยชน์เพื่อติดตามและประเมินผลการดูแล

การศึกษาฐานข้อมูลจากชุดข้อมูลมาตรฐาน 52 แฟ้มในกลุ่มโรคเบาหวานนั้น เนื่องจากมีอัตราความชุกสูงในคนไทยทำให้เกิดค่าใช้จ่ายและภาระโรคสูง ดังนั้นเพื่อศึกษาโครงสร้างตัวชี้วัดที่เหมาะสมโดยเจาะจงที่ฐานข้อมูลผู้ป่วยเป็นรูปแบบการวิจัยแบบผสมผสาน (mixed method research) คือการวิจัยเชิงปริมาณและเชิงคุณภาพ ทำการศึกษาฐานข้อมูลตั้งแต่เดือนตุลาคม พ.ศ. 2560 - กันยายน พ.ศ. 2561 จากหน่วยบริการระดับปฐมภูมิจังหวัดลพบุรีและจังหวัดเพชรบูรณ์ ผลการศึกษาพบว่า หน่วยบริการ A มีจำนวนข้อมูลผู้ป่วย 170 คน พบความครบถ้วนของข้อมูลร้อยละ 24.1-97.1 และความถูกต้องของข้อมูลร้อยละ 92.3-100 และหน่วยบริการ B มีจำนวนข้อมูลผู้ป่วย 8,836 คน พบความครบถ้วนของข้อมูลร้อยละ 0-81.4 และจากการตรวจสอบจากข้อมูลผู้ป่วยที่มีความครบถ้วน พบความถูกต้องของข้อมูลร้อยละ 67.9-100 ส่วนผลการสัมภาษณ์ผู้ปฏิบัติงานที่เกี่ยวข้องนั้น ทั้งสองหน่วยบริการได้ให้บทสรุปไปในทำนองเดียวกัน คือ การบันทึกข้อมูลที่ซ้ำซ้อนทำให้เกิดความคลาดเคลื่อนของข้อมูล และบันทึกในรูปแบบไม่ถูกต้อง ดังนั้น การพัฒนาระบบการรายงานผลลัพธ์ด้านความต่อเนื่องของการให้บริการผู้ป่วยสามารถสะท้อนความครบถ้วนและความถูกต้องของข้อมูลได้อีกด้วย
