บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อวิเคราะห์ต้นทุนการดูแลระยะกลางผู้ป่วยโรคหลอดเลือดสมอง ในหอผู้ป่วยในโรงพยาบาลตติยภูมิแห่งหนึ่ง ปีงบประมาณ 2561 โดยเป็นการศึกษาเชิงพรรณนาแบบภาคตัดขวาง ณ ช่วงเวลาใดเวลาหนึ่ง ดำเนินการเก็บข้อมูลระหว่างเดือนกรกฎาคมถึงธันวาคม พ.ศ. 2562 ผลการศึกษาพบว่า ผู้ป่วยโรคหลอดเลือดสมองเข้ารับบริการการดูแลระยะกลาง จำนวน 70 ราย (179 ครั้ง) ผู้ป่วยร้อยละ 71.4 ใช้สิทธิหลักประกันสุขภาพถ้วนหน้า/บัตรทอง ร้อยละ 25.7 ใช้สิทธิข้าราชการ และร้อยละ 2.9 ใช้สิทธิประกันสังคม มีจำนวนวันนอนทั้งหมด 1,417 วันนอน เฉลี่ย 7.9 วันนอนต่อราย มีต้นทุนรวมการให้บริการการดูแลระยะกลางผู้ป่วยโรคหลอดเลือดสมองเท่ากับ 2,293,622 บาท ต้นทุนสูงสุดเป็นต้นทุนค่าบริการพื้นฐาน 1,484,988 บาท รองลงมาเป็นต้นทุนค่าบริการทางการแพทย์ 807,472 บาท และ ต้นทุนค่าลงทุน 1,163 บาท เมื่อพิจารณาตามสิทธิการรักษาพยาบาล พบว่า ต้นทุนรวมมีค่าใกล้เคียงกันคือ สิทธิบัตรทอง สิทธิข้าราชการและสิทธิประกันสังคม มีต้นทุนเฉลี่ย 1,604; 1,656 และ 1,716 บาทต่อวันนอนตามลำดับ ต้นทุนรวมการรับบริการการดูแลระยะกลางผู้ป่วยโรคหลอดเลือดสมองเท่ากับ 4,722 บาทต่อราย เป็นต้นทุนทางตรงที่ไม่เกี่ยวกับการให้บริการทางการแพทย์ 4,182 บาทต่อราย และต้นทุนค่าเสียโอกาสเฉลี่ย 350-1,490 บาทต่อราย

การวิเคราะห์ต้นทุนโดยใช้เกณฑ์ผู้รับภาระต้นทุน ซึ่งเป็นต้นทุนภายในที่เกิดจากการจัดบริการของผู้ให้บริการและต้นทุนภายนอกที่เกิดกับผู้รับบริการ มีความสำคัญต่อการวางแผนกำหนดนโยบายสาธารณสุข ซึ่งระบบการเงินการคลังเป็นหัวใจของการพัฒนาระบบสุขภาพที่ดี ส่งผลให้ประชาชนมีสุขภาพดี สามารถเข้าถึงบริการสุขภาพที่มีความจำเป็นอย่างเป็นธรรมภายใต้ระบบหลักประกันสุขภาพถ้วนหน้าที่มีประสิทธิภาพในการใช้ทรัพยากร
