บทคัดย่อ

บัญชี จ(2) เป็นบัญชีย่อยที่เพิ่มเข้าสู่บัญชียาหลักแห่งชาติ ตั้งแต่ปี พ.ศ. 2550 เพื่อให้ผู้ป่วยที่มีความจำเป็นเฉพาะสามารถเข้าถึงยาราคาแพงได้อย่างเสมอภาคและเท่าเทียมในทุกสิทธิการรักษา โดยมีคณะอนุกรรมการพัฒนาบัญชียาหลักแห่งชาติภายใต้คณะกรรมการพัฒนาระบบยาแห่งชาติทำหน้าที่คัดเลือกรายการยาเข้าสู่บัญชี จ(2) ตลอดระยะเวลา 10 ปีที่ผ่านมานั้น กระบวนการคัดเลือกยาเข้าสู่บัญชี จ(2) รวมถึงนิยามและเกณฑ์ประกอบการพิจารณารายการยาในบัญชี จ(2) มีการพัฒนาอย่างต่อเนื่อง การศึกษานี้จึงมีวัตถุประสงค์เพื่อศึกษาหน่วยงาน กระบวนการ นิยาม/เกณฑ์การคัดเลือก ผลการดำเนินงาน รวมถึงปัญหาและอุปสรรคของการคัดเลือกยาจำเป็นที่มีราคาแพงเข้าสู่ยาบัญชี จ(2) ของบัญชียาหลักแห่งชาติ ตั้งแต่เริ่มมีบัญชี จ(2) ระหว่างปี พ.ศ. 2550-2561 โดยมีรูปแบบการศึกษาเป็นงานวิจัยเชิงพรรณนา ดำเนินการเก็บข้อมูลโดยทบทวนเอกสารและสัมภาษณ์กึ่งโครงสร้างกับผู้กำหนดนโยบาย รวมถึงผู้มีส่วนได้ส่วนเสียในกระบวนการคัดเลือกยาเข้าสู่บัญชี จ(2) และมีการวิเคราะห์ข้อมูลจากการสัมภาษณ์ด้วยใช้วิธีการวิเคราะห์เชิงเนื้อหา จากการศึกษาการเปลี่ยนแปลงที่สำคัญของกระบวนการและเกณฑ์คัดเลือกยาเข้าสู่บัญชี จ(2) ของคณะอนุกรรมการพัฒนาบัญชียาหลักแห่งชาติ ระหว่างปี พ.ศ. 2550–2561 พบว่ารายการยาบัญชี จ(2) เพิ่มขึ้นอย่างต่อเนื่องทุก 3 ปี ตามรอบการทำงานของคณะอนุกรรมการพัฒนาบัญชียาหลักแห่งชาติ โดยในปี พ.ศ. 2561 มีรายการยาทั้งหมดจำนวน 32 รายการ 40 ข้อบ่งใช้ ปัญหาอุปสรรคที่สำคัญในกระบวนการคัดเลือกยาประกอบด้วย 1) ข้อจำกัดด้านงบประมาณและกำลังคน 2) ข้อจำกัดด้านความถี่และระยะเวลาที่เปิดรับแบบเสนอยาเข้าสู่บัญชียาหลักแห่งชาติ และ 3) ปัญหาการตีความนิยามและเกณฑ์การตัดสินใจคัดเลือกยาเข้าสู่บัญชี จ(2) นอกจากนี้ ยังพบว่า ปัจจัยที่ส่งเสริมกระบวนการคัดเลือกยาเข้าสู่บัญชี จ(2) คือ การให้ความสำคัญกับการคัดเลือกยาบัญชี จ(2) เป็นลำดับแรกในการกระบวนการคัดเลือกยาเข้าบัญชียาหลักแห่งชาติ การทบทวนปัญหาและอุปสรรคและปรับปรุงการทำงานภายในของคณะอนุกรรมการพัฒนาบัญชียาหลักแห่งชาติ ดังนั้น การศึกษานี้จึงมีข้อเสนอแนะให้พัฒนาโครงสร้างของหน่วยงานที่รับผิดชอบและพัฒนาศักยภาพด้านกำลังคนเพื่อสนับสนุนการดำเนินงานทั้งระบบ ปรับปรุงกระบวนการดำเนินงาน ตลอดจนพัฒนาระบบกำกับติดตามและประเมินผลการดำเนินงานเพื่อเพิ่มประสิทธิภาพของกระบวนการคัดเลือกยาเข้าสู่บัญชี จ(2)
