การจัดเครือข่ายบริการสุขภาพเชิงพื้นที่เพื่อบรรเทาปัญหาการขาดแคลนบุคลากร

งานวิจัยนี้เป็นรายงานผลการวิเคราะห์เบื้องต้นของการจัดเครือข่ายบริการสุขภาพสำหรับกำลังคนด้านสุขภาพของโรงพยาบาลสังกัดสำนักงานปลัดกระทรวงสาธารณสุข โดยมีวัตถุประสงค์เพื่อศึกษาการขาดแคลนกำลังคนและการจัดเครือข่ายบริการสุขภาพ เพื่อเป็นข้อมูลสำหรับนโยบายการจัดสรรทรัพยากรบุคคลเชิงพื้นที่เพื่อลดปัญหาความขาดแคลนบุคลากรสุขภาพของประเทศ ผลการศึกษาเปรียบเทียบอัตรากำลังคนขั้นต่ำ ณ วันที่ 1 กันยายน พ.ศ. 2562 พบว่า สาขาวิชาชีพที่ขาดแคลนมากที่สุด คือ พยาบาล ทันตแพทย์และแพทย์ ซึ่งมีค่าเฉลี่ยแบบถ่วงน้ำหนักของ “ดัชนีความเข้มข้นของความขาดแคลน” ในระดับโรงพยาบาลทั่วประเทศ คือ ขาดแคลนเฉลี่ยร้อยละ 31, 29, และ 26 ตามลำดับ และประมาณร้อยละ 80 ของโรงพยาบาลสังกัดสำนักงานปลัดกระทรวงสาธารณสุขขาดแคลนพยาบาล ทันตแพทย์และแพทย์ ซึ่งจำนวนขาดแคลนเฉลี่ยแบบถ่วงน้ำหนักสำหรับพยาบาล คือ 68 คนต่อโรงพยาบาล และจำนวนขาดแคลนเฉลี่ยแบบถ่วงน้ำหนักสำหรับแพทย์และทันตแพทย์ คือ 10 และ 3 คนต่อโรงพยาบาล ตามลำดับ โดยขาดแคลนพยาบาลรวมทั้งสิ้นประมาณ 16,000 คน และขาดแคลนแพทย์และทันตแพทย์ประมาณ 3,400 และ 1,000 คน ตามลำดับ ทั้งนี้ พยาบาลจะมีความขาดแคลนรุนแรงในระดับปฐมภูมิและตติยภูมิ แต่ไม่ขาดแคลนในระดับทุติยภูมิ ส่วนแพทย์มีปัญหาขาดแคลนรุนแรงในโรงพยาบาลระดับทุติยภูมิ
การจัดเครือข่ายของโรงพยาบาลในระดับบริการเดียวกันจะไม่สามารถลดปัญหาความขาดแคลนได้ เนื่องจากมีความขาดแคลนบุคลากรสูง จึงไม่เพียงพอที่จะจัดสรรให้บรรเทาความขาดแคลนได้ แต่ถ้าสามารถบริหารจัดการเครือข่าย โดยรวมกำลังคนที่อยู่ในระดับบริการใกล้เคียงกัน เช่น รวมระดับปฐมภูมิ ทุติยภูมิระดับต้น และทุติยภูมิระดับกลาง ภายในพื้นที่อำเภอเดียวกัน จะสามารถลดปัญหาความขาดแคลนพยาบาลได้ โดยดัชนีความเข้มข้นของความขาดแคลนแบบถ่วงน้ำหนักของพยาบาลจะสามารถลดลงจากร้อยละ 69 ในโรงพยาบาลระดับปฐมภูมิ เหลือร้อยละ 25 เมื่อจัดเครือข่ายระดับอำเภอ โดยสามารถเกลี่ยจากระดับทุติยภูมิที่โดยเฉลี่ยไม่มีความขาดแคลน เพื่อช่วยพยาบาลในระดับปฐมภูมิและตติยภูมิ นอกจากนี้ การจัดเครือข่ายในระดับจังหวัดหรือเขตพื้นที่สุขภาพจะช่วยบรรเทาความขาดแคลนแพทย์ในระดับทุติยภูมิได้ อย่างไรก็ตาม การจัดเครือข่ายในรูปแบบต่าง ๆ ที่ศึกษาจะไม่สามารถลดปัญหาขาดแคลนโดยเฉลี่ยในสาขาวิชาชีพทันตแพทย์และเภสัชกร
