บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนค่ารักษาพยาบาลของโรงพยาบาลท่าสองยาง จังหวัดตาก ในมุมมองของผู้ให้บริการ โดยศึกษาข้อมูลย้อนหลัง ปีงบประมาณ 2561 ระหว่างวันที่ 1 ตุลาคม 2560 ถึง 30 กันยายน 2561 ใช้รูปแบบการวิเคราะห์ต้นทุนโรงพยาบาลและต้นทุนบริการด้วยวิธีมาตรฐาน (standard costing method) แบบบนลงล่าง (top-down method) และวิเคราะห์ต้นทุนรายผู้ป่วยด้วยวิธีจากล่างขึ้นบน (bottom-up method) จำแนกหน่วยต้นทุนเป็น 2 กลุ่มหลัก คือ หน่วยต้นทุนที่ให้บริการและหน่วยต้นทุนสนับสนุน ตามประเภทหน่วยงาน กระจายต้นทุนจากหน่วยต้นทุนสนับสนุนไปหน่วยต้นทุนหลักด้วยวิธีถอดสมการพร้อมกัน (simultaneous equation method) เกณฑ์การกระจาย ใช้ข้อมูลจากหน่วยสนับสนุนโดยตรงร่วมกับข้อมูลจากฐานข้อมูลของโรงพยาบาล และใช้โปรแกรมคอมพิวเตอร์สำเร็จรูป (Hospital Service Cost Estimate: HSCE) ในการประมวลผลข้อมูล

ผลการศึกษาพบว่าปีงบประมาณ 2561 โรงพยาบาลท่าสองยาง มีต้นทุนการให้บริการทั้งหมด 159,909,496 บาท มีสัดส่วนต้นทุนค่าแรง ค่าวัสดุ และค่าลงทุน เท่ากับ 56.7:32.4:10.9 ตามลำดับ มีต้นทุนผู้ป่วยนอกเฉลี่ยต่อครั้ง 615 บาท เมื่อจำแนกตามการมีสิทธิในสวัสดิการการรักษาพยาบาล อันได้แก่ สิทธิข้าราชการ สิทธิประกันสังคม สิทธิหลักประกันสุขภาพถ้วนหน้า ผู้มีปัญหาสถานะและสิทธิ สิทธิประกันสุขภาพต่างด้าว ชำระเงินเอง สิทธิตาม พรบ. ผู้ประสบภัยจากรถ และอื่นๆ พบว่าต้นทุนผู้ป่วยนอกเฉลี่ย เท่ากับ 747, 613, 599, 629, 617, 581, 1,175 และ 459 บาท ตามลำดับ ขณะที่ต้นทุนผู้ป่วยในเฉลี่ยต่อค่าน้ำหนักสัมพัทธ์ที่ปรับแล้ว 13,945 บาท เมื่อจำแนกตามสิทธิข้าราชการ สิทธิประกันสังคม สิทธิหลักประกันสุขภาพถ้วนหน้า ผู้มีปัญหาสถานะและสิทธิ สิทธิประกันสุขภาพต่างด้าว ชำระเงินเอง สิทธิตาม พรบ. ผู้ประสบภัยจากรถ และอื่นๆ เท่ากับ 15,330, 12,888, 13,792, 13,263, 14,429, 14,488, 10,840 และ 22,967 บาท ตามลำดับ ทั้งนี้ ผลการศึกษาข้างต้นแสดงให้เห็นต้นทุนการให้บริการของโรงพยาบาลท่าสองยางสูงกว่าเมื่อเทียบกับค่าบริการที่เรียกเก็บ และต้นทุนค่าแรงเป็นต้นทุนที่สูงที่สุด ผู้บริหารโรงพยาบาลควรนำข้อมูลเหล่านี้ไปปรับใช้ในการบริหารงานให้มีประสิทธิภาพยิ่งขึ้นและควรทำการศึกษาอัตราการคืนทุนในแต่ละสิทธิอย่างละเอียดต่อไป
