การเปรียบเทียบการบริหารจัดการและสิทธิประโยชน์ทางการแพทย์ระหว่างระบบประกันสังคมกับระบบหลักประกันสุขภาพถ้วนหน้า พ.ศ. 2563

บทคัดย่อ

แม้ว่าประเทศไทยจะมีระบบหลักประกันสุขภาพครอบคลุมคนไทยทุกคนตั้งแต่ พ.ศ. 2545 แต่ปัจจุบันยังมีระบบหลักประกันสุขภาพสำคัญ 3 ระบบใหญ่ ที่ยังมีความเหลื่อมล้ำเพราะมีความเป็นมา การบริหารจัดการ อัตราและวิธีการจ่ายเงินให้สถานพยาบาลที่แตกต่างกัน การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อเปรียบเทียบการบริหารจัดการและสิทธิประโยชน์ทางการแพทย์ระหว่างระบบประกันสังคมและระบบหลักประกันสุขภาพถ้วนหน้า โดยใช้วิธีการศึกษาเชิงพรรณนาด้วยการทบทวนวรรณกรรมและเอกสารที่เกี่ยวข้องกับการบริหารจัดการ สิทธิประโยชน์ทางการแพทย์และคลอดบุตร ณ วันที่ 1 มกราคม 2563 ผลจากการศึกษาพบว่ามีสิทธิประโยชน์ทางการแพทย์ที่แตกต่างกัน 11 สิทธิประโยชน์ มี 7 สิทธิประโยชน์ที่ระบบประกันสังคมไม่มี แต่ระบบหลักประกันสุขภาพถ้วนหน้ามี และมี 2 สิทธิประโยชน์ที่ระบบประกันสังคมมี แต่ระบบหลักประกันสุขภาพถ้วนหน้าไม่มี ส่วนสิทธิประโยชน์ทางการแพทย์ที่เหมือนกันแต่มีการบริหารจัดการที่แตกต่างกันนั้น พบว่ามี 3 สิทธิประโยชน์ที่บริหารจัดการเหมือนกันแต่มีอัตราการชดเชยที่ต่างกัน มี 7 สิทธิประโยชน์ที่ระบบประกันสังคมมีการบริหารจัดการเฉพาะหรือมีอัตราชดเชยให้สถานพยาบาลที่สูงกว่า และมี 13 สิทธิประโยชน์ที่ระบบหลักประกันสุขภาพถ้วนหน้ามีระบบบริหารจัดการเฉพาะหรือมีอัตราชดเชยให้สถานพยาบาลที่สูงกว่า และเมื่อเปรียบเทียบกับผลการศึกษาเมื่อ 1 มกราคม พ.ศ. 2554 พบว่า 10 ปีที่ผ่านมานั้น ระบบประกันสังคมได้ปรับวิธีการจ่ายเงินผู้ป่วยใน จากแบบเหมาจ่ายรายหัว (capitation) เป็นแบบกลุ่มวินิจฉัยโรคร่วม มีระบบยา จ (2) มีงบประมาณรายหัวและอัตราการชดเชยผู้ป่วยในตามกลุ่มวินิจฉัยโรคร่วมมากกว่าระบบหลักประกันสุขภาพถ้วนหน้า มีสิทธิประโยชน์เพิ่มขึ้น 27 รายการ มีอุปกรณ์และอวัยวะเทียมเพิ่มขึ้นจาก 81 รายการเป็น 291 รายการ ขณะที่ระบบหลักประกันสุขภาพถ้วนหน้ามีสิทธิประโยชน์เพิ่มขึ้น 30 รายการ มีจำนวนอุปกรณ์และอวัยวะเทียมเพิ่มขึ้นจาก 270 รายการเป็น 316 รายการ ความแตกต่างด้านสิทธิประโยชน์ทางการแพทย์ระหว่างสองระบบจากเดิม 15 รายการเหลือเพียง 11 รายการ และความแตกต่างด้านสิทธิประโยชน์ที่เหมือนกันแต่บริหารจัดการต่างกัน จากเดิม 26 รายการ เหลือเพียง 20 รายการ อย่างไรก็ตาม โดยรวมแล้วระบบหลักประกันสุขภาพถ้วนหน้ายังมีการบริหารจัดการเฉพาะโรคและสิทธิประโยชน์ทางการแพทย์ที่มากกว่าระบบประกันสังคม แต่มีงบประมาณรายหัวและอัตราการชดเชยผู้ป่วยในตามกลุ่มวินิจฉัยโรคร่วมที่ต่ำกว่าระบบประกันสังคม ข้อเสนอเชิงนโยบายที่สำคัญคือ ควรศึกษาเปรียบเทียบผลลัพธ์ของการให้บริการระหว่าง 2 ระบบประกันสุขภาพนี้ในด้านต่างๆ ต่อไป เช่น ด้านการเข้าถึงบริการ ต้นทุน-ประสิทธิผล คุณภาพการให้บริการ รวมทั้งศึกษาเกี่ยวกับการจัดตั้งหน่วยงานระดับประเทศเพื่อเป็นเจ้าภาพในการบริหารจัดการระบบหลักประกันสุขภาพของไทยให้ทัดเทียมกัน สมดังเจตนารมณ์ของรัฐธรรมนูญ
