บทคัดย่อ

โรคไข้เลือดออกเป็นโรคติดต่อที่เป็นปัญหาสาธารณสุขของภูมิภาคเขตร้อนและร้อนชื้นของโลก รวมทั้งประเทศไทย ติดต่อโดยยุงลายเป็นพาหะนำโรค อาการและอาการแสดงของโรคคล้ายกับโรคอื่นซึ่งยากต่อการวินิจฉัยโรค การใช้ชุดตรวจการติดเชื้อไวรัสเดงกี (DENV NS1) เพื่อตรวจคัดกรองจึงมีความสำคัญต่อการวินิจฉัยโรค การวิจัยเชิงสำรวจภาคตัดขวางนี้จึงมีวัตถุประสงค์เพื่อศึกษาผลของการใช้ชุดตรวจการติดเชื้อไวรัสเดงกี ต่อการรายงานโรคไข้เลือดออกตามระบบเฝ้าระวังโรค รง. 506 ในเขตกรุงเทพมหานครตั้งแต่ปี 2556 จนถึงปี 2560 กลุ่มตัวอย่าง คือ โรงพยาบาลในพื้นที่กรุงเทพมหานคร จำนวน 33 โรงพยาบาล เก็บข้อมูลเดือนมีนาคม 2562 โดยกลุ่มตัวอย่างตอบแบบสอบถามด้วยตนเอง วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติเชิงอนุมาน

ผลการศึกษา พบว่าแนวโน้มการใช้ชุดตรวจการติดเชื้อไวรัสเดงกีเพิ่มขึ้นตามสถานการณ์ของโรค ในปี 2558 มีการตรวจได้ผลบวกมากที่สุด (ร้อยละ 30.1) การรายงานมากสุดในปี 2558 จำนวน 11,881 ราย มีการรายงานโรคไข้เลือดออกในระบบเฝ้าระวัง รง. 506 น้อยกว่าการใช้ชุดตรวจที่ใช้ในโรงพยาบาลที่ได้ผลบวก (ร้อยละ 87.8) และมีการใช้ที่เพิ่มขึ้นเรื่อยๆ ทุกปี การรายงานโรคไข้เลือดออกที่มีการรายงานผลการใช้ชุดตรวจโรคติดเชื้อไวรัสเดงกีเพิ่มขึ้นตั้งแต่ปี 2557 ถึงปี 2560 รายงานโรคไข้เลือดออกในระยะเวลา 1 ถึง 2 วัน เพิ่มมากขึ้นตั้งแต่ปี 2558 ถึง 2560 การใช้ชุดตรวจการติดเชื้อไวรัสเดงกีมีความสัมพันธ์กับการรายงานชนิดโรคไข้เลือดออก (p < 0.001)

จากผลการศึกษามีข้อเสนอว่า โรงพยาบาลควรมีการพัฒนาระบบการรายงานผู้ป่วยในการกรอกข้อมูลผลการใช้ชุดตรวจ หรือผลการตรวจทางห้องปฏิบัติการ
