การกระจายร้านยากับความต้องการด้านสุขภาพในประเทศไทย

บทคัดย่อ

ร้านยาเป็นหน่วยบริการสุขภาพที่ให้บริการยาและสุขภาพเบื้องต้นในชุมชน การกระจายร้านยาในแต่ละพื้นที่อย่างเหมาะสมจะทำให้ประชาชนสามารถเข้าถึงบริการเภสัชกรรมชุมชนได้ง่ายขึ้น และมีความปลอดภัยจากการใช้ยามากขึ้น วัตถุประสงค์ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาสถานการณ์การกระจายร้านยากับสถานการณ์ความต้องการด้านสุขภาพในแต่ละเขตสุขภาพของประเทศไทย ระเบียบวิธีศึกษา ใช้วิธีวิเคราะห์ข้อมูลทุติยภูมิจากหน่วยงานที่เกี่ยวข้องในภาครัฐ ได้แก่ จำนวนประชากร จำนวนผู้ป่วยด้วยโรคที่ควรควบคุมด้วยบริการผู้ป่วยนอก (ambulatory care sensitive conditions: ACSC) ผลิตภัณฑ์มวลรวมรายจังหวัดต่อประชากร (gross provincial product per capita: GPP per capita) ข้อมูลร้านขายยาแผนปัจจุบัน (ข.ย. 1) ร้านยาคุณภาพ ร้านขายยาแผนปัจจุบันเฉพาะยาบรรจุเสร็จที่ไม่ใช่ยาอันตรายหรือยาควบคุมพิเศษ (ข.ย. 2) และสถานบริการสุขภาพอื่นๆ ทั้งที่รับผู้ป่วยไว้ค้างคืนและไม่รับผู้ป่วยไว้ค้างคืนของภาครัฐและเอกชน โดยใช้สถิติเชิงพรรณนา (descriptive statistics) ผลการศึกษา พบว่าในปี พ.ศ. 2559 มีร้านขายยาแผนปัจจุบัน (ข.ย. 1) ในประเทศไทย 16,053 ร้าน โดยเขตสุขภาพที่ 13 ซึ่งเป็นพื้นที่ของกรุงเทพมหานคร มีร้านยามากที่สุด (4,774 ร้าน) คิดเป็นร้อยละ 29.74 ของจำนวนทั้งประเทศ ในส่วนภูมิภาค พบว่าเขตสุขภาพที่ 4 มีร้านยามากที่สุด (1,780 ร้าน) คิดเป็นร้อยละ 11.09 และเขตสุขภาพที่ 3 มีร้านยาน้อยที่สุด (333 ร้าน) คิดเป็นร้อยละ 2.07 เมื่อพิจารณาจำนวนร้านยาต่อจำนวนประชากร พบว่าประเทศไทยมีร้านยา 2.47 ร้านต่อหมื่นประชากร และ 2.72 ร้านต่อผู้ป่วย ACSC (10,000 รายโรค) ทั้งนี้ จำนวนร้านยามีความสัมพันธ์เชิงบวกกับผลิตภัณฑ์มวลรวมรายจังหวัดต่อประชากรในแต่ละเขตสุขภาพ และเมื่อจำแนกร้านยาตามในและนอกเขตเทศบาล พบว่าเขตสุขภาพเกือบทั้งหมดมีร้านยาตั้งอยู่ในเขตเทศบาลมากกว่านอกเขตเทศบาล ผลการศึกษาแสดงให้เห็นว่าการกระจายร้านยาในประเทศไทยมีความเหลื่อมล้ำกันในแต่ละพื้นที่ ซึ่งอาจส่งผลให้การเข้าถึงยาและบริการเภสัชกรรมของประชาชนมีคุณภาพเท่าเทียมกัน
