สถานการณ์การบริโภคเครื่องดื่มแอลกอฮอล์และดัชนีความเสี่ยงต่อปัญหาแอลกอฮอล์ของจังหวัดในประเทศไทย: ข้อมูลการสำรวจ พ.ศ. 2560

การศึกษานี้มีวัตถุประสงค์เพื่อนำเสนอพฤติกรรมการดื่มแอลกอฮอล์และปัญหาที่เกิดขึ้นในประเทศไทย เป็นรายภูมิภาคและรายจังหวัด โดยใช้ค่าดัชนีความเสี่ยงต่อปัญหาแอลกอฮอล์รายจังหวัด (Provincial Alcohol Problem Index, PAPI) ใช้ข้อมูลทุติยภูมิจากการสำรวจพฤติกรรมการสูบบุหรี่และดื่มสุราของประชากร (สพบส.) พ.ศ. 2560 ซึ่งใช้แบบแผนการสุ่มตัวอย่างแบบ 2 ขั้นตอน (stratified 2-stage sampling) ได้กลุ่มตัวอย่างทั้งหมด 120,003 คน วิเคราะห์ข้อมูลแบบถ่วงน้ำหนัก นำเสนอเป็นค่าร้อยละและค่าดัชนีความเสี่ยงต่อปัญหาแอลกอฮอล์รายจังหวัด ซึ่งคำนวณจาก 5 ประเด็นสถานการณ์แอลกอฮอล์ในแต่ละจังหวัดเทียบกับค่าเฉลี่ยของประเทศ มีค่าอยู่ระหว่าง 0 (ความเสี่ยงต่ำที่สุด) ถึง 1 (ความเสี่ยงสูงที่สุด) ผลการศึกษาพบว่า ภาคเหนือและภาคตะวันออกเฉียงเหนือมีความชุกของนักดื่มทั้งในกลุ่มผู้ใหญ่ (ร้อยละ 35.4 และ 32.8 ตามลำดับ) และกลุ่มวัยรุ่น (ร้อยละ 19.0 และ 16.8 ตามลำดับ) สูงกว่าภูมิภาคอื่น ส่วนภาคกลางและภาคใต้มีสัดส่วนของการดื่มประจำและดื่มหนักสูงกว่าภูมิภาคอื่น จังหวัดที่มีดัชนีความเสี่ยงต่อปัญหาจากการดื่มแอลกอฮอล์สูงที่สุด ได้แก่ ลำปาง (0.72 คะแนน) เชียงราย (0.69 คะแนน) พิษณุโลก (0.65 คะแนน) จันทบุรี (0.64 คะแนน) และสุโขทัย (0.63 คะแนน) ดังนั้น สรุปได้ว่าปัญหาการดื่มแอลกอฮอล์ยังคงเป็นปัญหาสาธารณสุขที่สำคัญของประเทศไทย โดยมีความหลากหลายของประเด็นปัญหาที่สำคัญในแต่ละพื้นที่ โดยพื้นที่ที่ควรมีการแก้ไขโดยด่วน ได้แก่ จังหวัดลำปางและเชียงราย รวมถึงภาคเหนือและภาคตะวันออกเฉียงเหนือ ซึ่งการแก้ไขปัญหาแอลกอฮอล์ควรประกอบด้วยการสร้างกลไกขับเคลื่อนตามประเด็นปัญหาในระดับท้องถิ่น ร่วมกับการผลักดันเชิงนโยบายในระดับประเทศ
