ต้นทุนการให้บริการผู้ป่วยกลุ่มโรคจิตเภท พฤติกรรมแบบโรคจิตเภท และโรคหลงผิด

บทคัดย่อ

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาต้นทุนการให้บริการผู้ป่วยกลุ่มโรคจิตเภท พฤติกรรมแบบโรคจิตเภท และโรคหลงผิด โดยศึกษาข้อมูลย้อนหลังในปีงบประมาณ 2559 ทำการศึกษาในโรงพยาบาลกลุ่มเป้าหมายจำนวน 8 แห่ง ประกอบด้วยโรงพยาบาลศูนย์ 1 แห่ง โรงพยาบาลทั่วไป 3 แห่ง และโรงพยาบาลชุมชน 4 แห่ง คัดเลือกกลุ่มตัวอย่างโดยวิธีการคัดเลือกแบบเฉพาะเจาะจงจากปัจจัยความพร้อมของระบบข้อมูลที่เกี่ยวข้อง คือ มีระบบข้อมูลบริการ และมีข้อมูลค่าใช้จ่ายที่เพียงพอต่อการคำนวณต้นทุนเฉลี่ยต่อครั้งของการให้บริการผู้ป่วย จากนั้นจึงคำนวณต้นทุนบริการตามคู่มือการศึกษาต้นทุนของกระทรวงสาธารณสุขและปรับใช้อัตราส่วนต้นทุนต่อรายได้ (cost to charge ratio) ในระดับผู้ป่วยรายบุคคล

ผลการศึกษา พบว่า ต้นทุนการให้บริการที่สูงที่สุดคือค่าบุคลากร รองมาคือค่าวัสดุ และต้นทุนการใช้สินทรัพย์ ส่วนในรายละเอียดของต้นทุนเฉลี่ยของการให้บริการผู้ป่วยกลุ่มโรคจิตเภท พฤติกรรมแบบโรคจิตเภท และโรคหลงผิด พบว่า โรงพยาบาลศูนย์มีต้นทุนการบริการผู้ป่วยนอกเฉลี่ย 831 บาทต่อครั้ง ขณะที่โรงพยาบาลทั่วไปมีต้นทุนบริการผู้ป่วยนอกเฉลี่ย 483 - 1,030 บาทต่อครั้ง และโรงพยาบาลชุมชนมีต้นทุนการให้บริการผู้ป่วยนอกเฉลี่ย 189 - 567 บาทต่อครั้ง

อย่างไรก็ตาม การศึกษานี้มีข้อมูลต้นทุนของโรงพยาบาลกลุ่มเป้าหมายเพียง 8 แห่ง และมีโรงพยาบาลศูนย์เพียงแห่งเดียวซึ่งต้นทุนที่ได้จากการศึกษานี้อาจไม่สามารถเป็นตัวแทนที่ดีในแต่ละประเภทโรงพยาบาลได้ ดังนั้น จึงอาจนำไปปรับใช้กับโรงพยาบาลที่มีบริบทคล้ายกันเท่านั้น
