ความเป็นธรรมในการได้รับบริการสุขภาพช่องปากของประชากรไทย: การวิเคราะห์ผลการสำรวจอนามัยและสวัสดิการ พ.ศ. 2560

ประเทศไทยประสบความสำเร็จในการจัดให้มีระบบหลักประกันสุขภาพถ้วนหน้าแก่ประชาชนตั้งแต่ พ.ศ. 2545 ส่งผลให้ประชากรเกือบทั้งหมดมีสิทธิในสวัสดิการการรักษาทางทันตกรรมที่จัดให้โดยรัฐ แต่จากผลการสำรวจระดับประเทศที่ผ่านมานั้น การได้รับบริการสุขภาพช่องปากของประชาชนยังคงมีสัดส่วนที่ต่ำ รวมถึงยังมีปัญหาความเป็นธรรมในการได้รับบริการสุขภาพช่องปาก การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อวิเคราะห์ความเป็นธรรมในการได้รับบริการสุขภาพช่องปากของประชากรไทยผ่านสัดส่วนและรูปแบบการได้รับบริการสุขภาพช่องปาก รวมถึงความจำเป็นด้านสุขภาพช่องปากที่ไม่ได้รับการตอบสนองของประชากรกลุ่มต่างๆ โดยใช้ข้อมูลทุติยภูมิจากการสำรวจอนามัยและสวัสดิการ พ.ศ. 2560 ของสำนักงานสถิติแห่งชาติ ผลการศึกษาแสดงให้เห็นว่า ประชากรเพียงร้อยละ 9.6 เท่านั้นที่ได้รับบริการสุขภาพช่องปากในรอบ 12 เดือนที่ผ่านมา โดยรูปแบบการรับบริการเป็นไปเพื่อการรักษาถึงร้อยละ 81.6 ของการรับบริการทั้งหมด สาเหตุหลักที่ประชาชนไม่ไปรับบริการสุขภาพช่องปากก็คือ ช้าและรอนาน ในส่วนของความจำเป็นด้านสุขภาพช่องปากที่ไม่ได้รับการตอบสนองนั้น พบว่า ประชากรร้อยละ 1.4 มีความต้องการในการเข้ารับบริการสุขภาพช่องปากที่ไม่ได้รับการตอบสนอง โดยกลุ่มที่ไม่มีสิทธิสวัสดิการ กลุ่มผู้สูงอายุ และกลุ่มเศรษฐฐานะต่ำที่สุดมีความจำเป็นทางสุขภาพช่องปากที่ไม่ได้รับการตอบสนองมากกว่ากลุ่มประชากรอื่นๆ ถึงสองเท่า แต่กลับเป็นกลุ่มที่ได้รับบริการสุขภาพช่องปากน้อยที่สุด ผลการสำรวจแสดงให้เห็นถึงความไม่เป็นธรรมในการได้รับบริการสุขภาพช่องปากโดยเฉพาะในกลุ่มประชากรต่างสิทธิสวัสดิการและต่างเศรษฐฐานะ สะท้อนให้เห็นความจำเป็นที่จะต้องพัฒนานโยบายสุขภาพช่องปากเพื่อให้ตอบสนองต่อความต้องการของประชาชน และสร้างความเป็นธรรมของระบบการให้บริการสุขภาพช่องปาก โดยเฉพาะการพัฒนาความรอบรู้ด้านสุขภาพช่องปาก การสร้างความกลมกลืนระหว่างสามระบบสวัสดิการรักษาพยาบาลของรัฐ และการขยายขอบเขตการให้บริการเพื่อให้ประชาชนเข้าถึงบริการที่จำเป็นได้มากขึ้น
