ภาระทางเศรษฐกิจจากการดูแลรักษาผู้ป่วยต่างด้าว: กรณีศึกษาโรงพยาบาลพหลพลพยุหเสนา จังหวัดกาญจนบุรี

จังหวัดกาญจนบุรีเป็นหนึ่งในจังหวัดชายแดนที่มีประชากรต่างด้าวอาศัยอยู่หนาแน่น ทำให้โรงพยาบาลพหลพลพยุหเสนาซึ่งเป็นโรงพยาบาลประจำจังหวัดต้องให้การดูแลรักษาผู้ป่วยต่างด้าวจำนวนมากมาอย่างต่อเนื่อง การศึกษานี้มีวัตถุประสงค์ที่จะศึกษาสถานการณ์และผลกระทบของการให้บริการผู้ป่วยต่างด้าวต่อระบบบริการและค่าใช้จ่ายของโรงพยาบาลพหลพลพยุหเสนา โดยเป็นการศึกษาเชิงปริมาณของผู้ป่วยต่างด้าวที่มารักษาในโรงพยาบาลพหลพลพยุหเสนาทั้งผู้ป่วยนอกและผู้ป่วยใน ในปีงบประมาณ 2560 เก็บข้อมูลโดยการสัมภาษณ์ ใช้แบบสอบถามและการสืบค้นข้อมูลการมารับบริการย้อนหลัง สุ่มตัวอย่างตามสะดวก มีผู้เข้าร่วมการศึกษาทั้งหมด 519 คน ประกอบด้วยผู้ป่วยนอก 412 คนและผู้ป่วยใน 107 คน โดยในกลุ่มผู้ป่วยนอก 412 คน มี 305 คนที่มารับบริการผู้ป่วยนอกอย่างเดียว และมี 107 คนที่เคยมีประวัติการนอนโรงพยาบาลในช่วงปีที่ผ่านมา ผลการศึกษาพบว่า กลุ่มตัวอย่างผู้ป่วยนอกร้อยละ 18.4 และกลุ่มตัวอย่างผู้ป่วยในร้อยละ 38.3 ไม่มีสิทธิประกันสุขภาพ เมื่อจำแนกผู้ป่วยตามกลุ่มโรคที่มีต้นทุนสูงที่สุดพบว่า ผู้ป่วยกลุ่มโรคมะเร็งมีสัดส่วนสูงที่สุดในกลุ่มตัวอย่างผู้ป่วยนอก [63,459 บาทต่อคนต่อปี (SD 82,872)] และกลุ่มผู้ป่วยโรคติดต่อมีสัดส่วนสูงสุดในกลุ่มตัวอย่างผู้ป่วยใน [43,100 บาทต่อครั้ง (SD 53,384)] ในแง่ต้นทุนค่ารักษาพยาบาลและค่ารักษาพยาบาลตามราคาโรงพยาบาล ผู้ป่วยนอกมีต้นทุนเฉลี่ย 15,757 บาทต่อคนต่อปี ผู้ป่วยในมีต้นทุนเฉลี่ย 35,280 บาทต่อคนต่อครั้ง ของโรงพยาบาลพหลพลพยุหเสนา โรงพยาบาลสมุทรสาคร โรงพยาบาลกระทุ่มแบน และโครงการวิจัย ค่าน้ำหนักสัมพัทธ์ของค่าใช้จ่ายมากกว่าเท่ากับ 2 หน่วยของโรงพยาบาลพหลพลพยุหเสนา โรงพยาบาลสมุทรสาคร โรงพยาบาลกระทุ่มแบน และโครงการวิจัย ร้อยละ 19.2, 5.3, 2.4, และ 23.4 ตามลำดับ ค่าน้ำหนักสัมพัทธ์ของค่าใช้จ่ายมากกว่าเท่ากับ 4 หน่วย ร้อยละ 5.8, 1.9, 1.2, และ 3.7 ตามลำดับ 5 อันดับสูงสุดของกลุ่มโรคผู้ป่วยนอก คือมะเร็ง การบาดเจ็บ โรคไม่ติดต่อ ทารกแรกเกิดที่มีปัญหา และการฝากครรภ์และการคลอด ตามลำดับ 5 อันดับสูงสุดของฝ่ายการแพทย์ที่รับผู้ป่วยใน ได้แก่ อายุรกรรม ศัลยกรรม สูติกรรม ศัลยกรรมกระดูกและกุมารเวชกรรม ตามลำดับ
ข้อมูลการศึกษานี้ เป็นตัวอย่างที่สนับสนุนให้มีการพิจารณาเชิงนโยบายเพื่อแก้ปัญหาภาระค่าใช้จ่ายของโรงพยาบาลในจังหวัดชายแดน เช่น การเสนอนโยบายความร่วมมือกับประเทศเพื่อนบ้านในการพัฒนาขีดความสามารถของสถานพยาบาล การทบทวนราคาบัตรประกันสุขภาพต่างด้าวของกระทรวงสาธารณสุขให้เหมาะสมกับต้นทุนการรักษาพยาบาล และที่สำคัญควรมีนโยบายปรับการจัดสรรงบประมาณชดเชยให้แก่สถานพยาบาลในกรณีที่ดูแลคนต่างด้าวและไม่สามารถเรียกเก็บค่าใช้จ่ายได้เพียงพอ
