บทคัดย่อ

ภูมิหลังและเหตุผล: นโยบาย "30 บาทรักษาได้ทุกที่ด้วยบัตรประชาชนใบเดียว" ซึ่งริเริ่มในปี พ.ศ. 2567 เป็นนวัตกรรมเชิงซับซ้อนที่อาศัยโครงสร้างพื้นฐานดิจิทัลสุขภาพเพื่อขยายการเข้าถึงบริการสุขภาพ อย่างไรก็ตาม ความสำเร็จของนโยบายมิได้ขึ้นกับการออกแบบเพียงอย่างเดียว แต่ขึ้นกับความสามารถของหน่วยบริการในการนำนโยบายไปปฏิบัติจริง

วัตถุประสงค์: เพื่อประเมินปัจจัยกำหนดการนำนโยบายไปปฏิบัติและวัดระดับการยอมรับนโยบาย (adoption) ผ่านการเปลี่ยนแปลงของอัตราการใช้บริการผู้ป่วยนอกของโรงพยาบาลสวรรค์ประชารักษ์

ระเบียบวิธีศึกษา: ใช้ระเบียบวิธีวิจัยแบบผสมผสานชนิดคู่ขนาน (convergent parallel mixed methods) ดำเนินการระหว่างเดือนพฤษภาคม–สิงหาคม พ.ศ. 2568 เก็บข้อมูลผ่านการสัมภาษณ์เชิงลึกรายบุคคลกับผู้ปฏิบัติงาน 14 คน จาก 7 กลุ่มวิชาชีพ คัดเลือกแบบเจาะจง โดยใช้แนวคำถามกึ่งโครงสร้างที่พัฒนาจาก Consolidated Framework for Implementation Research ครอบคลุม 35 ใน 43 องค์ประกอบของกรอบ CFIR 2.0 ในการศึกษาเชิงปริมาณได้วิเคราะห์ข้อมูลอนุกรมเวลารายสัปดาห์ของการใช้บริการผู้ป่วยนอกกลุ่มโรคเบาหวานชนิดที่ 2 (E11) และโรคความดันโลหิตสูง (I10–I15) เปรียบเทียบปี พ.ศ. 2567 (36 สัปดาห์) กับเส้นฐาน พ.ศ. 2562–2566 (240 สัปดาห์) ด้วยสถิติ one-sample t-test, Welch's t-test, Cohen's d, Bootstrap CI (10,000 resamples) และ Wilcoxon signed-rank test

ผลการศึกษา: ด้านคุณภาพ พบว่าร้อยละ 71.4 ของ CFIR constructs (25 จาก 35 องค์ประกอบ) เป็นปัจจัยเอื้อต่อการนำนโยบายไปปฏิบัติ โดยปัจจัยหลัก ได้แก่ ความสามารถปรับตัว (adaptability +2) การมีส่วนร่วมของผู้นำ (leadership engagement +2) ความพร้อมของบุคลากร (self-efficacy, stage of change +2) และกระบวนการนำนโยบายไปปฏิบัติที่มีระบบ (planning, executing, reflecting +2) ขณะที่อุปสรรคเชิงโครงสร้างที่สำคัญ (–2) ได้แก่ คุณภาพหลักฐาน ต้นทุนและทรัพยากร และความไม่สอดคล้องกับกระบวนการทำงานจริง อัตราการใช้บริการปี 2567 เพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติในทั้งโรคเบาหวาน (เพิ่มขึ้นร้อยละ 19.8 จาก 870 เป็น 1,042 ครั้ง/สัปดาห์; Cohen's d = 1.040, p < 0.001) และโรคความดันโลหิตสูง (เพิ่มขึ้นร้อยละ 16.9 จาก 1,450 เป็น 1,695 ครั้ง/สัปดาห์; Cohen's d = 0.896, p < 0.001) เป็นผลลัพธ์ขนาดใหญ่ และการวิเคราะห์ความไวยืนยันความแข็งแกร่งของผลลัพธ์ อย่างไรก็ตาม การเปรียบเทียบกับ baseline 2 ปีล่าสุดพบว่ากลุ่มโรคความดันโลหิตสูงไม่แตกต่างอย่างมีนัยสำคัญ (p = 0.100) ซึ่งบ่งชี้ถึงแนวโน้มระยะยาวก่อนนโยบาย

สรุป: ความสำเร็จของการนำนโยบายไปปฏิบัติมิได้เกิดจากตัวนโยบายเพียงอย่างเดียว แต่เกิดจากความสอดคล้องระหว่างความแข็งแกร่งของ "คน" และ "กระบวนการ" ภายในองค์กรกับโครงสร้างการสนับสนุนในระดับนโยบาย การที่อุปสรรคหลักทั้งสี่ประการอยู่นอกเหนืออำนาจของโรงพยาบาล สะท้อนว่าการขยายผลระดับประเทศจำเป็นต้องแก้ไขช่องว่างด้านการเบิกจ่าย ต้นทุนการดำเนินการ และความสอดคล้องของระบบดิจิทัลกับบริบทจริงก่อน อย่างไรก็ตาม การศึกษานี้มีข้อจำกัดด้านการอ้างอิงผล เนื่องจากเป็นการศึกษาสถานพยาบาลเดียวที่เป็นโรงพยาบาลนำร่องและมีความพร้อมด้านดิจิทัลสูง ประกอบกับการวิเคราะห์เชิงปริมาณไม่สามารถควบคุมแนวโน้มระยะยาวและตัวแปรกวนภายนอกได้อย่างสมบูรณ์
