บทคัดย่อ

ภูมิหลังและเหตุผล:  การกระจายยาและผลิตภัณฑ์สุขภาพที่ไม่ปลอดภัยในชุมชนในเขตภาคเหนือเป็นปัญหาที่เกิดขึ้นอย่างต่อเนื่อง และส่งผลกระทบต่อสุขภาพของประชาชน การบริหารจัดการเพื่อการเฝ้าระวังฯ เป็นไปตามยุทธศาสตร์ชาติด้านการส่งเสริมการใช้ยาอย่างสมเหตุผล การวิจัยนี้มีวัตถุประสงค์เพื่อศึกษารูปแบบการดำเนินการเฝ้าระวังยาและผลิตภัณฑ์สุขภาพที่ไม่ปลอดภัยในชุมชนและบทบาทของภาคีเครือข่ายใน 3 ประเด็นปัญหายาของภาคเหนือ คือ ยาชายแดน ยาชุมชน และการใช้ยาปฏิชีวนะในสัตว์น้ำ

ระเบียบวิธีศึกษา:  การวิจัยเชิงคุณภาพ เก็บข้อมูลจากการอภิปรายกลุ่มเพื่อถอดบทเรียนจากผู้มีส่วนเกี่ยวข้องในการดำเนินการเฝ้าระวังฯ รวบรวมข้อมูลระหว่างเดือนพฤศจิกายน 2566 ถึง พฤษภาคม 2567 ผู้ให้ข้อมูลประกอบด้วยตัวแทนแกนนำเภสัชกรปฐมภูมิและภาคีเครือข่ายในพื้นที่ รวม 56 คน ยาชายแดนศึกษาในเขตจังหวัดเชียงรายและพะเยา ยาชุมชนศึกษาในเชียงใหม่และแม่ฮ่องสอน และการใช้ยาปฏิชีวนะในสัตว์น้ำ ศึกษาในจังหวัดพะเยา วิเคราะห์ข้อมูลเชิงคุณภาพและหาข้อสรุปแบบอุปนัย

ผลการศึกษา:  พบรูปแบบการดำเนินการเฝ้าระวังฯ 3 รูปแบบที่แตกต่างกันตามกรณีศึกษา ทุกรูปแบบประกอบด้วย 4 ประเด็นหลัก (themes) แสดงขั้นตอนการดำเนินการเฝ้าระวังฯ ได้แก่  (1) การสำรวจสถานการณ์ในพื้นที่ (2) การสื่อสารข้อมูลสถานการณ์ไปยังเครือข่าย (3) การพัฒนาเครื่องมือและแนวทางการดำเนินการเพื่อการเฝ้าระวัง และ (4) การประสานงานเพื่อแก้ไขปัญหาร่วมกับชุมชน โดยกรณีศึกษายาชายแดน ทีมเภสัชกรปฐมภูมิสำรวจสถานการณ์ที่พบการลักลอบนำเข้ายาอันตรายและผลิตภัณฑ์ที่ไม่มีตรา อย. (สำนักงานคณะกรรมการอาหารและยา) รับรอง ฉลากไม่ถูกต้อง ผ่านด่านและตลาดนัดชายแดนกระจายสู่รถเร่และออนไลน์ สื่อสารข้อมูลสถานการณ์ข้อมูลผลิตภัณฑ์ที่ไม่ปลอดภัยและผลกระทบกับภาคีเครือข่าย มีการพัฒนาเครื่องมือโดยการกำหนดชนิดผลิตภัณฑ์เป้าหมายและใช้ไลน์กลุ่มแจ้งเตือนภัยอย่างรวดเร็ว มีการประสานงานร่วมกับชุมชนและหน่วยงานฝ่ายความมั่นคงและฝ่ายปกครองในการตรวจจับที่จุดผ่อนปรนทางการค้าและร่วมออกข้อบัญญัติท้องถิ่นควบคุม  สำหรับกรณีศึกษายาชุมชน เภสัชกรปฐมภูมิสำรวจสถานการณ์ผ่านการรับเรื่องร้องเรียนของสภาองค์กรของผู้บริโภค (สอบ.) มีการตรวจร้านชำและเยี่ยมบ้านร่วมกับเครือข่ายสหสาขาวิชาชีพ นำข้อมูลมาวิเคราะห์เส้นทางกระจายยาที่ไม่ปลอดภัย สื่อสารข้อมูลสถานการณ์ไปยังภาคีเครือข่ายเพื่อสะท้อนปัญหา พร้อมกำหนดบทบาทหน้าที่การเฝ้าระวังร่วมกัน พัฒนาเครื่องมือเก็บข้อมูล สร้างสื่อที่เหมาะกับบริบทพื้นที่และเครื่องมือส่งสัญญาณ (trigger tool) ประสานงานร่วมกับชุมชนผ่านนโยบายการพัฒนาคุณภาพชีวิตระดับอำเภอ (พชอ.) และนโยบายการใช้ยาอย่างสมเหตุผล (Rational Drug Use Community) พร้อมยกระดับแกนนำในการเฝ้าระวังครัวเรือน สำหรับกรณีศึกษาการใช้ยาปฏิชีวนะในสัตว์น้ำนั้น เภสัชกรปฐมภูมิร่วมกับ สอบ. และสำนักงานสาธารณสุขจังหวัด (สสจ.) สำรวจสถานการณ์เชิงลึกการใช้ยาปฏิชีวนะและการจัดการน้ำตลอดเส้นทางในฟาร์มปลานิล เพื่อตรวจวิเคราะห์สารปฏิชีวนะตกค้างในเนื้อปลา น้ำ และอาหารสัตว์ จากนั้นทำการสื่อสารข้อมูลสถานการณ์แก่ประมงจังหวัด สหกรณ์ และกลุ่มเกษตรกร  ร่วมกันกำหนดมาตรการแนวทางแก้ไขเชิงระบบ พัฒนาเครื่องมือเป็นสื่อเอกสารและคู่มือมาตรฐานการปฏิบัติทางการเกษตรที่ดี และประสานงานร่วมกับชุมชนโดยใช้กลไกสหกรณ์ควบคุมคุณภาพ ผลักดันนโยบายอาหารปลอดภัยและมอบป้ายรับรองเพื่อสร้างกลไกตลาดปลอดสาร รูปแบบการดำเนินการดังกล่าวเกิดเป็นตัวอย่างการจัดการการดื้อยาปฏิชีวนะในชุมชนที่ดี

ข้อยุติ: การเฝ้าระวังยาและผลิตภัณฑ์สุขภาพที่ไม่ปลอดภัยในชุมชน 3 กรณีศึกษาของภาคเหนือ พบว่ามีรูปแบบที่แตกต่างกันตามบริบท ส่วนการดำเนินการมีขั้นตอนสำคัญเหมือนกัน หน่วยงานภาคสาธารณสุขเป็นผู้ขับเคลื่อนหลัก โดยมีเภสัชกรปฐมภูมิเป็นผู้ประสานงานและมีหน่วยงานการปกครอง หน่วยงานความมั่นคง องค์กรปกครองส่วนท้องถิ่น เครือข่ายภาคประชาชน หน่วยงานการศึกษาและผู้ประกอบการภาคเอกชนเป็นผู้ร่วมดำเนินการเฝ้าระวังยาและผลิตภัณฑ์สุขภาพที่ไม่ปลอดภัยในชุมชน
