บทคัดย่อ

หลักการและเหตุผล: การสร้างสิ่งแวดล้อมที่เอื้อต่อการส่งเสริมสุขภาพเป็นกลยุทธ์สำคัญในการดูแลสุขภาพเด็กวัยเรียน กระทรวงศึกษาธิการ ได้ออกประกาศ พ.ศ. 2563 เรื่องมาตรการและแนวทางการสร้างเสริมความรอบรู้ด้านสุขภาพช่องปาก และสำนักงานคณะกรรมการการศึกษาขั้นพื้นฐาน (สพฐ.) ได้ออกประกาศ พ.ศ. 2564 ว่าด้วยการจำหน่ายอาหารและเครื่องดื่มที่ส่งเสริมสุขภาพ

วัตถุประสงค์: เพื่อศึกษาระดับการรับรู้ประกาศทั้งสองฉบับและศึกษาความสัมพันธ์ระหว่างการรับรู้ประกาศกับการจัดสิ่งแวดล้อม ความรอบรู้ พฤติกรรมและสภาวะสุขภาพของนักเรียน

ระเบียบวิธีศึกษา: เป็นการวิจัยผสมผสาน โดยการศึกษาภาคตัดขวาง ประกอบด้วย 1) การศึกษาเชิงปริมาณ โดยสำรวจผู้อำนวยการและครูอนามัยจาก 81 โรงเรียน และนักเรียนชั้นประถมศึกษาปีที่ 6 จำนวน 715 คน ใช้แบบสอบถาม วิเคราะห์ด้วยสถิติ chi-square และ binary logistic regression 2) การศึกษาเชิงคุณภาพ โดยสัมภาษณ์เชิงลึกผู้บริหารและครูอนามัย 30 คน และผู้แทนเขตพื้นที่การศึกษา 5 คน วิเคราะห์เนื้อหาด้วย thematic analysis

ผลการศึกษา: พบว่าผู้บริหารหรือครูอนามัยมีการรับรู้ประกาศกระทรวงศึกษาธิการ และประกาศ สพฐ. ร้อยละ 49.4 และ 72.8 ตามลำดับ โรงเรียนที่ผู้บริหาร/ครูอนามัยรับรู้ประกาศ สพฐ. มีการจัดสิ่งแวดล้อมที่เอื้อต่อสุขภาพมากกว่า นักเรียนในโรงเรียนดังกล่าวมีความรอบรู้ด้านอาหารร้อยละ 80 ขึ้นไปมากกว่า (odds ratio=2.18, 95% confidence interval: 1.43-3.32) มีพฤติกรรมแปรงฟันหลังอาหารกลางวันทุกวันมากกว่า (OR=2.15, 95% CI: 1.37-3.37) และดื่มน้ำอัดลมน้อยกว่า โรงเรียนในเครือข่ายมีการควบคุมการจำหน่ายขนมและเครื่องดื่มมากกว่าโรงเรียนนอกเครือข่าย (p<0.01) จากการสัมภาษณ์เชิงลึกพบว่า การรับรู้และดำเนินงานตามประกาศขึ้นกับการสนับสนุนของผู้บริหาร ระบบติดตามและการประสานงานกับหน่วยงานสาธารณสุข

สรุป: การรับรู้ประกาศ สพฐ. พ.ศ. 2564 สัมพันธ์กับการจัดสิ่งแวดล้อมที่เอื้อต่อสุขภาพในโรงเรียนและพฤติกรรมสุขภาพของนักเรียนในทิศทางที่ดีกว่า ผลการศึกษาชี้ให้เห็นว่าการเผยแพร่นโยบายอย่างทั่วถึงจะเป็นปัจจัยสนับสนุนสำคัญต่อการสร้างสิ่งแวดล้อมส่งเสริมสุขภาพในโรงเรียน จึงควรมีการประชาสัมพันธ์ประกาศทั้งสองฉบับอย่างต่อเนื่อง พร้อมจัดทำคู่มือปฏิบัติที่ชัดเจนและกลไกติดตามที่เป็นระบบ
