Anti-Myelin Oligodendrocyte Glycoprotein Antibodies associated Transverse Myelitis Patient : A Case Study
Main Article Content
Abstract
A 24-year-old male without an underlying disease presented with progressive paraparesis for 12 days with urinary disturbance requiring a Foley catheter for 8 days. His significant neurological deficit included motor power grade 0 of both lower extremities, which is upper motor neuron type, and impairment of pinprick sensation up to T4 dermatome. MRI imaging of his whole spine showed a long segmental hyperintense lesion in T2-weighted image involving spinal cord at C2 to L1 levels. Many tests were done to find the cause of transverse myelitis in this patient. At last, anti-myelin oligodendrocyte glycoprotein antibodies (anti-MOG Ab) were found in his serum. The initial treatment of intravenous highdose steroid for 5 days had not improved his motor power, so plasmapheresis was arranged for 5 times in 10 days, resulting in improvement of his motor power of right and left legs to grade II and IV, respectively. Then a prednisolone taper followed. 1 month after the treatment, the patient had visited OPD, walking normally, and the Foley catheter was removed. At 6 months, anti-MOG antibodies test will be repeated for evaluation of risk of disease relapse.
