บทคัดย่อ
โรคเรื้อนเป็นโรคติดต่อที่ต้องเฝ้าระวังตามพระราชบัญญัติโรคติดต่อ พ.ศ. 2558 จึงต้องมีระบบการติดตาม ตรวจสอบและจัดเก็บข้อมูลอย่างต่อเนื่อง ปัจจุบันระบบเฝ้าระวังโรคเรื้อนติดตามข้อมูลต่อเนื่องในฐานข้อมูลโรคเรื้อน สถาบันราชประชาสมาสัย (LEP database) อีกทั้งมีการรายงานผู้ป่วยโรคเรื้อนรายใหม่ในระบบรายงาน 506 กองระบาดวิทยา การรายงานโรคเรื้อนในระบบรายงาน 506 ยังไม่เป็นที่ใช้แพร่หลาย การศึกษานี้จัดทำขึ้น เพื่อพรรณนาระบบเฝ้าระวังโรคเรื้อนของประเทศไทย ในปี 2566 และเพื่อประเมินคุณลักษณะเชิงปริมาณของระบบเฝ้าระวังโรคเรื้อนในระบบรายงาน 506 ตั้งแต่วันที่ 1 มกราคม - 8 ตุลาคม 2566 โดยใช้ LEP database เป็นฐานข้อมูลอ้างอิงผลการศึกษาพบว่า ระบบรายงานโรคเรื้อน LEP database และระบบรายงาน 506 มีวัตถุประสงค์และตัวแปรในการรายงานที่แตกต่างกัน ระบบรายงาน 506 รายงานตั้งแต่ผู้ป่วยสงสัย เพื่อตรวจจับการระบาดในขณะที่ LEP database รายงานผู้ป่วยยืนยันและตัวแปรที่จำเพาะต่อโรคเรื้อน เช่น ระดับความพิการ การรายงานผู้ป่วยโรคเรื้อนในระบบรายงาน 506 ปัจจุบันยังคงมีการรายงานเข้ามาน้อย มีความไวร้อยละ 30 ส่งผลต่อความเป็นตัวแทนของข้อมูลที่ไม่สะท้อนลักษณะของผู้ป่วยโรคเรื้อนรายใหม่เมื่อใช้ LEP database เป็นฐานข้อมูลอ้างอิง อย่างไรก็ตามพบค่าพยากรณ์บวกสูง ร้อยละ 93.8 หากแต่ยังคงมีความล่าช้าของการรายงานผู้ป่วยรายใหม่เข้าสู่ระบบรายงาน 506 ทั้งนี้สถาบันราชประชาสมาสัย จึงควรมีการผลักดันการรายงานโรคเรื้อนในระบบรายงาน 506 และจัดทำฐานข้อมูลระบบเฝ้าระวังผู้ป่วยโรคเรื้อนรายใหม่ที่ประกอบไปด้วยรายงานผู้ป่วยใหม่จากระบบรายงาน 506 ที่ได้รับการตรวจสอบว่าเป็นผู้ป่วยยืนยันแล้ว และข้อมูลจำเพาะของโรคเรื้อน รวมถึงรายละเอียดของผู้สัมผัสใกล้ชิดจากฐานข้อมูล LEP database ซึ่งจะทำให้เกิดฐานข้อมูลในการเฝ้าระวังผู้ป่วยโรคเรื้อนที่ทันเวลา ถูกต้อง และครบถ้วน นำไปสู่การควบคุมโรคที่ต่อเนื่องเป็นระบบมีประสิทธิภาพ และสามารถกำจัดโรคเรื้อนในประเทศไทยได้ในที่สุด
