การศึกษาเปรียบเทียบผลลัพธ์ตัวชี้วัดบริการ NCD Clinic ในโรงพยาบาลสังกัดสำนักงานปลัดกระทรวง ของจังหวัดฉะเชิงเทรา ปีงบประมาณ พ.ศ. 2567

บทคัดย่อ
การวิจัยนี้เป็นแบบผสมผสาน (mixed-methods research) ชนิด convergent parallel design มีวัตถุประสงค์เพื่อ (1) เปรียบเทียบผลลัพธ์ตัวชี้วัดบริการด้านการควบคุมระดับน้ำตาลในเลือด (HbA1c) ของผู้ป่วยโรคเบาหวานอายุต่ำกว่า 60 ปีและอายุ 60 ปีขึ้นไป ระหว่างโรงพยาบาลในจังหวัดฉะเชิงเทรา (2) ศึกษารูปแบบการดำเนินงาน NCD Clinic Plus ด้านระบบบริการและบุคลากร และ (3) เปรียบเทียบระหว่างผลลัพธ์การควบคุมระดับน้ำตาลในเลือด HbA1c กับปัจจัยด้านระบบบริการและนโยบาย การศึกษาเชิงปริมาณเป็นการศึกษาจากประชากรทั้งหมด 43,873 คน (total population study) โดยไม่ใช้วิธีการสุ่มตัวอย่าง เก็บข้อมูลผู้ป่วยโรคเบาหวานที่มารับบริการ จากฐานข้อมูล Data Exchange ใน Health Data Center ระหว่างเดือนตุลาคม 2566 ถึงมีนาคม 2567 วิเคราะห์ด้วยสถิติเชิงพรรณนา และเปรียบเทียบความแตกต่างด้วย Kruskal–Wallis test พร้อมรายงานขนาดอิทธิพล (effect size: η²) ส่วนการศึกษาเชิงคุณภาพใช้แบบสัมภาษณ์เชิงลึกแบบตัวต่อตัวกับพยาบาลประจำคลินิก โรงพยาบาลละ 1 คน รวม 11 แห่ง เครื่องมือผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้เชี่ยวชาญ (IOC = 1.00) และวิเคราะห์เชิงเนื้อหาเพื่อนำมาบูรณาการกับผลเชิงปริมาณ ผลการศึกษาพบว่ามีความแตกต่างของการควบคุมระดับน้ำตาลในเลือด HbA1c ระหว่างโรงพยาบาลอย่างมีนัยสำคัญทางสถิติ โดยข้อมูลเชิงคุณภาพพบว่าระบบบริการแบบสหวิชาชีพ การใช้แนวทางการดูแลผู้ป่วยเบาหวานชนิดที่ 2 ให้เข้าสู่โรคเบาหวานระยะสงบ (DM Remission) และการสนับสนุนเชิงนโยบายมีความสัมพันธ์กับผลลัพธ์ที่ดี นอกจากนี้การพัฒนาระบบสารสนเทศ การใช้เทคโนโลยีดิจิทัล และการส่งเสริมการมีส่วนร่วมของชุมชนจะช่วยยกระดับคุณภาพการดูแลผู้ป่วย NCD ได้อย่างยั่งยืน
