ผลการใช้แนวทางปฏิบัติการเจาะเลือดส่งตรวจเพาะเชื้อของบุคลากรในโรงพยาบาลจุฬาภรณ์ จังหวัดนครศรีธรรมราช

บทคัดย่อ
การวิจัยเชิงทดลองแบบกลุ่มเดียววัดผลก่อน-หลังนี้ มีวัตถุประสงค์เพื่อศึกษาผลของการใช้แนวทางปฏิบัติการเจาะเลือดส่งตรวจเพาะเชื้อในโรงพยาบาลจุฬาภรณ์ จังหวัดนครศรีธรรมราช โดยเปรียบเทียบความรู้ของบุคลากร อัตราการปนเปื้อนเชื้อจุลชีพ และความพึงพอใจต่อการใช้แนวทางปฏิบัติ กลุ่มตัวอย่างเป็นพยาบาลวิชาชีพในกลุ่มงานการพยาบาล โรงพยาบาลจุฬาภรณ์ จังหวัดนครศรีธรรมราช จำนวน 26 คน เลือกแบบเจาะจง ดำเนินการศึกษาในช่วงเดือนพฤศจิกายน 2566–มกราคม 2567 เครื่องมือที่ใช้ในการเก็บข้อมูลประกอบด้วย แบบบันทึกข้อมูลทั่วไป แบบประเมินความรู้เกี่ยวกับการเจาะเลือดส่งตรวจเพาะเชื้อ แบบประเมินความพึงพอใจต่อการใช้แนวทางปฏิบัติ และแบบเก็บข้อมูลผลรายงานเพาะเชื้อจากห้องชันสูตร ซึ่งผ่านการตรวจสอบความตรงเชิงเนื้อหาโดยผู้ทรงคุณวุฒิ 3 ท่าน และมีค่าความเชื่อมั่นของแบบประเมินความรู้และความพึงพอใจ (Cronbach’s alpha) เท่ากับ 0.90 วิเคราะห์ข้อมูลด้วยสถิติพรรณนา ได้แก่ จำนวน ร้อยละ ค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน และสถิติอนุมาน ได้แก่ สถิติทีแบบจับคู่ (paired t-test) และการทดสอบของฟิชเชอร์ (Fisher’s exact test) กำหนดระดับนัยสำคัญที่ p-value < 0.05 ผลการศึกษา พบว่า คะแนนเฉลี่ยความรู้หลังการใช้แนวทางปฏิบัติสูงกว่าก่อนการใช้แนวทางอย่างมีนัยสำคัญทางสถิติ ( = 12.1 ± 1.7 เทียบกับ
= 14.7 ± 0.7; t = −7.5, p-value < 0.001) อัตราการปนเปื้อนเชื้อจุลชีพในเลือดลดลงจากร้อยละ 5.7 (5/87 ตัวอย่าง) เหลือร้อยละ 0 (0/78 ตัวอย่าง) อย่างมีนัยสำคัญทางสถิติ (p-value = 0.039) และความพึงพอใจต่อการใช้แนวทางปฏิบัติอยู่ในระดับมาก (
= 4.8 ± 0.3) ผลการศึกษาชี้ให้เห็นว่า การนำแนวทางปฏิบัติร่วมกับการอบรมและการนิเทศติดตามสามารถเพิ่มความรู้ของบุคลากร ลดอัตราการปนเปื้อนเชื้อ และส่งเสริมความปลอดภัยของผู้ป่วยได้อย่างมีประสิทธิภาพ ควรบูรณาการแนวทางดังกล่าวเป็นมาตรฐานการปฏิบัติงาน และพัฒนาเป็นระบบติดตามคุณภาพอย่างต่อเนื่องในหน่วยบริการ
