การประเมินนโยบาย “คนไทยห่างไกลโรคไม่ติดต่อเรื้อรัง (NCDs)” กรณีศึกษาจังหวัดชลบุรี ปี 2568

บทคัดย่อ
โรคไม่ติดต่อเรื้อรัง (NCDs) เป็นปัญหาสาธารณสุขสำคัญ โดยเฉพาะในพื้นที่เขตพัฒนาพิเศษภาคตะวันออก (EEC) ซึ่งมีลักษณะเศรษฐกิจอุตสาหกรรม การเคลื่อนย้ายแรงงานสูง และวิถีชีวิตที่ไม่เอื้อต่อการดูแลสุขภาพ แม้ว่านโยบาย “คนไทยห่างไกลโรคไม่ติดต่อเรื้อรัง” จะถูกขับเคลื่อนอย่างต่อเนื่อง แต่ยังขาดหลักฐานเชิงประจักษ์ในระดับพื้นที่เกี่ยวกับช่องว่างระหว่างความครอบคลุมของบริการและผลลัพธ์ด้านสุขภาพ การศึกษานี้มีวัตถุประสงค์เพื่อประเมินนโยบายดังกล่าวในจังหวัดชลบุรี การศึกษานี้เป็นการวิจัยแบบผสมผสาน (Mixed-Research Method) ภายใต้กรอบการประเมินนโยบายสุขภาพ โดยใช้การประเมินเชิงรูปแบบและเชิงสรุปผล เพื่อวิเคราะห์ทั้งกระบวนการดำเนินงานและผลลัพธ์ของนโยบายในจังหวัดชลบุรี ซึ่งครอบคลุมพื้นที่ตัวอย่าง 33 ตำบลจาก 11 อำเภอ กลุ่มตัวอย่างประกอบด้วยผู้มีส่วนได้ส่วนเสีย 44 คน และผู้ป่วย 148 คน วิเคราะห์ข้อมูลเชิงปริมาณด้วยสถิติเชิงพรรณนา และข้อมูลเชิงคุณภาพด้วยการวิเคราะห์แก่นสาระ (Thematic analysis) ผลการศึกษาพบว่า นโยบายสามารถขับเคลื่อนได้จริงในระดับพื้นที่ โดยมีการปรับรูปแบบจากการบริหารแบบลำดับชั้นไปสู่การดำเนินงานผ่านเครือข่ายความร่วมมือ ซึ่งอาศัยทุนทางสังคมเป็นกลไกสำคัญในการเชื่อมโยงการทำงาน อย่างไรก็ตาม พบความย้อนแย้งเชิงระบบที่สำคัญ คือ แม้ตัวชี้วัดด้านความครอบคลุมของบริการจะบรรลุเป้าหมายในระดับสูง (ร้อยละ 100) แต่ผลลัพธ์ด้านสุขภาพของประชาชนยังไม่ดีขึ้นอย่างมีนัยสำคัญ สะท้อนถึงช่องว่างระหว่างความรู้ ทัศนคติ และพฤติกรรมสุขภาพ รวมถึงข้อจำกัดของระบบข้อมูลและรูปแบบบริการที่ไม่สอดคล้องกับวิถีชีวิตของแรงงานในพื้นที่อุตสาหกรรม ข้อค้นพบนี้ชี้ให้เห็นว่า ความสำเร็จของนโยบายไม่ได้ขึ้นอยู่กับการเพิ่มความครอบคลุมของบริการเพียงอย่างเดียว แต่ขึ้นอยู่กับความสามารถของระบบในการเชื่อมโยงการดำเนินงาน พฤติกรรมประชาชน และข้อมูลสุขภาพให้เกิดผลลัพธ์เชิงสุขภาพอย่างแท้จริง ดังนั้น ควรพัฒนาบริการเชิงรุกที่ยืดหยุ่น เช่น การให้บริการนอกเวลา การจัดบริการในสถานประกอบการ และการใช้เทคโนโลยีติดตามสุขภาพ ควบคู่กับการพัฒนาระบบข้อมูลแบบบูรณาการ เพื่อเพิ่มประสิทธิภาพของนโยบายในบริบทพื้นที่ EEC อย่างยั่งยืน
