ปัญหายารักษาโรคสมาธิสั้นขาดคราว: แนวทางการจัดการสำหรับกุมารแพทย์ในประเทศไทย

การขาดคราวของยารักษาโรคสมาธิสั้น โดยเฉพาะกลุ่มยาออกฤทธิ์กระตุ้นระบบประสาท (stimulant) เป็นปัญหาสำคัญที่กระทบความต่อเนื่องของการรักษา สมรรถนะการเรียน และความมั่นคงทางอารมณ์ของเด็กและครอบครัว บทความนี้ได้อธิบายภาพรวมการขาดคราวและความท้าทายเชิงระบบในบริบทไทย พร้อมสรุปภาพรวมยารักษาโรคสมาธิสั้นที่ใช้จริงในประเทศ ได้แก่ methylphenidate ชนิดออกฤทธิ์สั้นและออกฤทธิ์นาน (Ritalin®, Rubifen®, methylphenidate GPO, Ritalin® LA, Concerta®, methylphenidate Sandoz®) ทั้งนี้บริษัทที่เคยนำเข้ายากลุ่ม non-stimulant ได้แก่ atomoxetine ได้ยุติการนำเข้ามาประมาณ 1 ปีที่ผ่านมา จึงทำให้ไม่มียา non-stimulant ที่ผ่านการรับรองโดยองค์การอาหารและยาในประเทศขณะนี้ และการเตรียมพร้อมต่อการมียา lisdexamfetamine ในอนาคต ส่วนท้ายเป็นข้อเสนอเชิงปฏิบัติแบบวางแผนล่วงหน้าที่ผู้เขียนเสนอสำหรับคลินิกพัฒนาการที่มี case coordinator หรือ case manager โดยใช้การคัดแยกผู้ป่วยเป็นกลุ่มสีเพื่อกำหนดระดับการติดตาม การสื่อสาร และการเตรียมยาอย่างเป็นระบบ พร้อมตารางประกอบเพื่อช่วยตัดสินใจในคลินิก
