Prevalence of Colistin-resistant organisms isolated from specimens of patients admitted to Ratchaburi Hospital between the years 2020 and 2024

Abstract
Currently, colistin-resistant bacteria are increasingly being reported around the world. which includes Thailand from such situation this made the researchers interested in studying the prevalence of colistin -resistant bacteria in Ratchaburi hospitals. By studying historical data during 2020-2024 and has classified the germs into three groups: Pseudomonas aeruginosa resistant to colistin, Acinetobacter baumannii resistant to colistin, and Enterobacterales resistant to colistin it was found that Pseudomonas aeruginosa resistant to colistin tended to decrease. But while Acinetobacter baumannii that are resistant to colistin,and Enterobacterales that are resistant to colistin are increasing, From a study of the reasons why colistin resistance decreased in Pseudomonas aeruginosa and increased in Acinetobacter baumannii and Enterobacterales It is a matter of factors in using colistin.
When studied by separating the microorganisms according to the type of specimen, it was found that Pseudomonas aeruginosa resistant to colistin and Acinetobacter baumannii resistant to colistin were found very high in sputum samples. While colistin -resistant Enterobacterales were most abundant in urine samples, this was consistent with the environmental conditions in which they were found. And when studying by separating the detected organisms by department, it was found that Pseudomonas aeruginosa resistant to colistin and Enterobacterales resistant to colistin were found the most in the internal medicine and surgery departments. While colistin-resistant Acinetobacter baumannii was most prevalent in ICUs.
Tracking trends in colistin resistance among pathogens is crucial for surveillance and prevention of infection spread, as well as for appropriate drug use. Therefore, the hospital should track the colistin usage rate each year and compare usage trends to guide the development of appropriate policies, planning, and decision-making regarding medication use.
