Development of Nursing Practice Guidelines for Stroke Patients in the Rehabilitation Phase at Kamalasai Hospital

This action research aimed to develop a nursing practice guideline for patients with stroke in the rehabilitation phase at Kamalasai Hospital, Kalasin Province. The study participants consisted of four groups: (1) key informants involved in the guideline development phase, (2) the guideline development team, (3) stroke patients in the rehabilitation phase receiving services at the outpatient department, and (4) caregivers of the patients. The research instruments included the Barthel Index for assessing activities of daily living, the WHOQOL-BREF for assessing quality of life, a satisfaction questionnaire, and focus group discussion records. Quantitative data were analyzed using descriptive statistics and paired t-test, while qualitative data were analyzed using content analysis.
The results revealed that:
Regarding the situation and needs of stroke rehabilitation care in the outpatient department, the service lacked standardized nursing practice guidelines. Patient assessment mainly focused on physical aspects, with insufficient holistic assessment covering psychological, social, and quality of life dimensions. Additionally, workload and time constraints limited continuity of care and follow-up, and caregivers lacked adequate knowledge and skills for home-based rehabilitation.
The developed nursing practice guideline consisted of four main components: (1) holistic assessment, (2) care planning, (3) nursing implementation, and (4) follow-up and referral.
The evaluation of the guideline implementation showed that patients had significantly improved scores in activities of daily living and quality of life (p < .05). Caregivers reported a high level of satisfaction and perceived that the guideline improved the systematic and continuity of patient care.
In conclusion, the developed nursing practice guideline effectively enhanced the quality and continuity of care for stroke patients in the rehabilitation phase and should be further implemented in similar healthcare settings.
