The development of a nursing model to prevent and control the spread of antimicrobial-resistant infections in Yang Si Surat Hospital

Abstract

   

     The objectives of this research were to develop and evaluate a nursing model for the prevention and control of antimicrobial-resistant (AMR) pathogen transmission at Yang Sisurat Hospital, Maha Sarakham Province. This study employed an action research design. The sample, selected through purposive sampling, consisted of 31 professional nurses working in the Inpatient, Outpatient, and Emergency departments. Research instruments included an AMR prevention knowledge assessment, an observational checklist for clinical practice compliance, and a satisfaction questionnaire. Quantitative data were analyzed using frequency, percentage, mean, and Paired t-test, while qualitative data were analyzed using content analysis.

     The results revealed that :The developed nursing model, titled the "SMART Model," comprises five key components: S (Standard): Correct and standardized clinical practice guidelines. M (Method Precaution): Transmission prevention methods, including personal protective equipment (PPE) usage, hand hygiene, environmental cleaning, and soiled linen management. A (Accountability): Clear identification of responsible personnel and role assignments. R (Referral System): Systems for the transfer and surveillance of infected patients. T (Technique Success): Follow-up through specimen cultures and clinical supervision according to the guidelines. Evaluation results indicated that: After implementing the model, staff knowledge and understanding of AMR prevention and control increased significantly (p < 0.001), with mean scores rising from 8.20 (SD = 1.87) to 13.40 (SD = 1.21). Correct practice among staff significantly improved from 53.36% to 85.81% . Overall satisfaction with the developed model was at a high level (52.7%)
