Development of a Screening Model for Chronic Kidney Disease in Mahasarakham Hospital

This Participatory Action Research aimed to: 1) analyze the current situation of chronic kidney disease (CKD) screening; 2) develop a CKD screening model; and 3) evaluate the developed screening model at Mahasarakham Hospital. Participants were selected by purposive sampling and consisted of 20 professional nurses working at the Outpatient Department (OPD) and 150 patients aged 20–89 years who attended the OPD clinic between October 2024 and September 2025. The study employed Kemmis and McTaggart’s (1988) PAOR framework. Research instruments included: 1) a screening and referral toolkit comprising a clinical workflow, a CKD screening form, and a patient education handbook; and 2) data collection tools for nurses and patients. The instruments were validated by three experts (IOC = 0.67–1.00) and tested for reliability (Cronbach’s alpha = 0.82–0.91). Data were analyzed using descriptive statistics, the Friedman test, and the Wilcoxon Signed Ranks Test with Bonferroni correction.
The results revealed that nurses expressed the highest level of satisfaction with the developed model and tools across all aspects (median satisfaction: 90.00%–94.29%), confirming its practical feasibility in the medical OPD and other departments. Regarding clinical outcomes, patients' eGFR levels showed statistically significant improvement (p < .05). Although the median eGFR slightly decreased in the 6th month (90.78) compared to the 3rd month (91.37), it remained significantly higher than the baseline before implementation (p < .016). These findings suggest that the developed screening model not only systematically identifies high-risk groups but also facilitates continuous care to preserve kidney function. Therefore, this model should be expanded to other healthcare service units.
