Objectives: To compare basic life support (BLS) knowledge and skills before and after training within groups, and after training between a group taught with a 4-step photograph-based CPR media and a group taught with the 7-step CPR media of the National Institute for Emergency Medicine.
Methods: A quasi-experimental two-group pretest–posttest non-equivalent control group design was used. Sixty-two service users aged 18–60 years at Na Dun Hospital, Maha Sarakham Province, were enrolled between June 2022 – July 2023. Sample size was determined by an a priori power analysis for two independent means. Eligible participants were recruited consecutively and allocated by time period to a control group (7-step media, n = 31) and an intervention group (4-step media, n = 31). Both groups received identical lecture, demonstration and manikin return-demonstration training of equal duration. Data were collected with a BLS knowledge test (KR-20 = .88) and a skills checklist and analysed using paired and independent t-tests.
Results: Post-training knowledge and skill scores increased significantly in both groups (p < .001). Post-training knowledge did not differ between the intervention (M = 6.81, SD = 0.75) and control groups (M = 6.77, SD = 1.20; p = .875), whereas post-training skill scores were significantly higher in the intervention group (M = 9.27 vs. 8.53, SD = 1.13 each; t = 2.58, p = .012, Cohen’s d = 0.65).
Conclusion: The 4-step photograph-based media achieved knowledge outcomes comparable to the standard media and was associated with higher skill scores, making it a suitable option for community CPR education in rural settings. Content should be updated to the 2025 resuscitation guidelines, and randomized trials with objective CPR-quality and retention measures are recommended.
