This action research aimed to develop a health behavior modification model for individuals at risk of diabetes mellitus in Na Dun Subdistrict, Maha Sarakham Province, Thailand. The study was conducted from October 2022 to September 2023 in four phases: data assessment, model development, implementation, and reflection. Purposive sampling was used to recruit 60 individuals at risk of diabetes mellitus with fingertip blood glucose (DTX) levels of 100–125 mg/dL, 60 family caregivers, and 30 multidisciplinary healthcare professionals and health network members. Research instruments included semi-structured interviews, risk screening forms, health literacy and health behavior assessments based on the 3E2S principles, focus group discussions, and a satisfaction questionnaire. Quantitative data were analyzed using descriptive statistics and paired t-tests, while qualitative data were analyzed using content analysis.
The findings revealed that overweight or obesity (80.00%), excessive waist circumference (70.00%), and lack of regular exercise (100.00%) were major risk factors. The developed model, called the Na Dun Five-Base 3E2S Model, integrated individual risk assessment, family caregiver involvement, and five community activity bases: healthy eating, exercise, emotional well-being, smoking cessation, and alcohol reduction. Activities were conducted every Friday using a six-step behavior modification process. After implementation, health literacy scores improved significantly across all domains (p < .001), with access to health information increasing from 2.72 to 3.68. The proportion of participants who did not exercise regularly decreased from 100.00% to 0.00%, while the proportion who added seasonings to food decreased from 68.75% to 0.00%. Overall satisfaction was high (Mean = 4.27, SD = 0.38).
In conclusion, the Na Dun Five-Base 3E2S Model was suitable for the community context and showed promising short-term improvements in health literacy and health behaviors among individuals at risk of diabetes mellitus. It may be adapted for use in communities with similar characteristics.
