Gastric Pneumatosis ในทารกแรกเกิดก่อนกำหนด และน้ำหนักตัวน้อยกว่าอายุครรภ์

ภาวะ Gastric Pneumatosis คือการมีลมรั่วมาในผนังของกระเพาะอาหาร ภาะดังกล่าวสามารถเกิดได้จากหลายปัจจัย ไม่ว่าจะเป็นการใส่ Noninvasive ventilation, การใส่สายให้อาหาร หรือปัจจัยที่มีความรุนแรงได้แก่ การติดเชื้อ ซึ่งมีความคล้ายคลึงกับภาวะ Pneumatosis Intestinalis ที่พบได้ใน Necrotizing Enterocolitis ซึ่งเป็นโรคที่รุนแรงที่พบได้ในทารกแรกเกิดก่อนกำหนดในผู้ป่วยรายนี้เป็นทารกแรกเกิดก่อนกำหนดอายุครรภ์ 36 สัปดาห์ และมีน้ำหนักตัวน้อยกว่าอายุครรภ์แรกรับมิปัญหาหายใจเหนื่อย มีข้อบ่งชี้ในการใส่ Noninvasive ventilation ในรูปแบบ CPAP หลังจากใส่อุปกรณ์ดังกล่าวทารกมีอาการหายใจเหนื่อยลดลง แต่ว่าในเวลาต่อมามีอาการท้องอืดมากขึ้น และดูดได้เลือดจากสายให้อาหารทางปาก (Orogastric tube) ได้ทำการส่งตรวจ plain fim abdomen พบ Gastric distension with preumatosis intestinalis บริเวณกระเพาะอาหาร เค้าได้กับ Gastric Pneumatosis จึงได้ทำการรักษาโดยให้ยาฆ่าเชื้อ และงดอาหาร พบว่าทารกตอบสนองต่อการรักษา และมีภาพ plain fim abdomen ที่ดีขึ้นในเวลาถัดมา
