Nursing Care for Patients with Upper Gastrointestinal Bleeding with Hypovolemic Shock and Comorbidity : 2 Case Studies

Abstract

This study aims to examine disease progression, nursing diagnosis, nursing care plan, and the results of two patients with upper Gastrointestinal bleeding with Hypovolemic Shock and Comorbidity hospitalized as outpatients from March to December 2022 at Khon Kaen Hospital. The study relies on patients’ information, related articles and papers, and healthcare professionals. The data analysis relies on Gordon's Functional Health Patterns in order for health assessment and nursing diagnosis indication in the condition length of treatment. The result of the study shows that there were different symptoms in three periods: Initial period 1) Hypovolemic Shock due to the bleeding in the upper Gastrointestinal. Inpatient period 2) Risk of complications from shock 3) Risk of Hypoxia due to decreased lung function 4) Anxiety about illness and treatment 5) Electrolyte Imbalance due to blood loss. 6) Urinary tract infection 7) Fatigue and unstable consciousness reducing self-care Follow-up period 8) Risk of re-bleeding due to pathology of the disease and improper behavior 9) Anemia condition reducing endurance 10) Risk of the spread of COVID-19 toward family members.

With those conditions, nurses take an important role in rescuing patients. It is essential to have the ability to diagnose the level of severity, the accuracy for categorizing patients, priority management skills, the ability of problem analysis, the ability to manage nursing plans and following up the treatments until patients’ illness is cured.
