Development of Participatory Recovered Intermediate Care Model Khok Phra Sub-district, Kantarawichai District, Mahasarakham Province

Abstract

Objectives : 1)to analyze the situation of an intermediate care service in Khok Phra Sub-district 2)to develop a participatory recovered intermediate care model. 3)to evaluate the effectiveness of a participatory recovered intermediate care model in Khok Phra Sub-district, Kantarawichai District, Maha Sarakham Province.

Design and methodology: This action research was carried out throughout 1st October 2022 and 30th September 2023. Target population and co-investigator were 1) 8 intermediate care patients 2) 28 multidisciplinary of public health personnel included physician, registered nurse, physical therapist, nutritionist, public health technical officer and village. health volunteers. Content analysis was used for qualitative data and statistics for quantitative were number, percentage, mean, standard deviation, maximum and minimum.

Results: Development of a participatory recovered intermediate care model activities were committee appointment, database preparation and application for new case registration, define the roles and responsibilities of multidisciplinary of public health personnel clearly in recovered intermediate care model, training program with 3doctor conception for village. health volunteer education and multidisciplinary of public health personnel, follow up with a standard criterion, once a week follow up by village. health volunteer, coordination helps via an application, situation analysis, information feedback to problem solving and guideline for cyclic working. These were done until reach the completed of a participatory recovered intermediate care model. 3) The result of independence in intermediate care patients was increased from 50%to 87.50%

Conclusion: The development of a participatory recovered intermediate care model worked with standard criterion of patient care by multidisciplinary of public health personnel and community. The effective result of model has been appropriated with community context.
