Traumatic Axial Ulnar Disruption of the Carpus Review of Literature and a Case Report

Abstract
Axial ulnar disruption of the carpus เป็น injury ที่พบ rare มาก รายงานครั้งแรกในปี ค.ศ. 1901 โดย Oberst หลังจากนั้นมี report ใน world literature อีก 27 ราย ในประเทศไทยยังไม่มีผู้ใดรายงานไว้ ผู้ป่วยที่นำเสนอเป็นชาย อายุ 27 ปี ประสบอุบัติเหตุจราจรมี axial disruption ของทั้งกระดูก carpus และ metacarpus โดยแนวการแยกอยู่ระหว่างกระดูก capitate และ hamate ทำให้มีการเคลื่อนที่ของ metacarpal bone อันที่ 4,5 พร้อมทั้งกระดูก hamate, pisiform ไปทางด้าน ulnar นอกจากนี้ยังมี dislocation ของกระดูก triquetrum เข้าไปแทรกระหว่าง capitate และ hamate และมี scapholunocapitate dissociation ซึ่ง Axial ulnar disruption ชนิดนี้ยังไม่มีผู้ใดรายงานมาก่อนได้ทำการรักษาโดย open reduction and Kirschner wires fixation และทำ scapholunocapitate fusion ในเวลาต่อมาผลการรักษาและติดตามผู้ป่วย 4 ปี ผู้ป่วยสามารถกลับไปประกอบอาชีพเดิมได้ ไม่มี pain แต่ ROM และ grip strength ลดลง
