บทคัดย่อ
Autoimmune/inflammatory syndrome induced by adjuvants (ASIA) เป็นกลุ่มอาการที่รวมอาการที่เกิดจากความผิดปกติในระบบภูมิคุ้มกันหลังจากได้รับสารเสริมฤทธิ์ ซึ่งประกอบด้วยอาการที่พบในกลุ่มอาการต่างๆ ที่เคยมีการกล่าวถึงก่อนหน้านี้ ได้แก่ ปรากฏการณ์หลังจากฉีดวัคซีน (post vaccination phenomenon), macrophagic myofasciitis syndrome, Gulf war syndrome และ siliconosis โดยปัจจัยเสี่ยงในการเกิดความผิดปกติหลังสัมผัสสารเสริมฤทธิ์ คือ ผู้ที่เคยมีประวัติการเกิดปฏิกิริยาหรือความผิดปกติหลังได้รับสารเสริมฤทธิ์มาก่อน ผู้ที่มีโรคทางระบบภูมิคุ้มกัน มีประวัติอาการแพ้หรือโรคภูมิแพ้ และมีแนวโน้มการเกิดโรคทางระบบภูมิคุ้มกัน ในปัจจุบันมีรายงานการศึกษาผู้ป่วยมีอาการ ASIA หลังได้รับสารเสริมฤทธิ์เพิ่มมากขึ้น อย่างไรก็ตามจากผลการศึกษาในผู้ป่วยโรคข้อและรูมาติสซั่มที่ผ่านมายังไม่พบลักษณะอาการ ASIA หรือมีอาการกาเริบของโรคข้อและรูมาติสซั่มภายหลังจากการได้รับวัคซีน ในปัจจุบันการสืบค้นทางห้องปฏิบัติการที่จาเพาะเจาะจงในการวินิจฉัยกลุ่มอาการ ASIA ยังไม่แน่ชัด รวมถึงการรักษายังไม่มีการรักษาเฉพาะเจาะจง การรักษาส่วนใหญ่จึงมุ่งเน้นการรักษาตามอาการเป็นส่วนใหญ่ และในกรณีที่อาการ ASIA เกิดจากการเสริมใส่อุปกรณ์ที่มีคุณสมบัติเป็นสารเสริมฤทธิ์ ควรแนะนาให้ผู้ป่วยถอดสิ่งเสริมใส่ที่นั้นออกเพื่อลดหรือหยุดยั้งอาการที่เกิดขึ้น
