Cryptococcosis ในไขกระดูกผู้ป่วยติดเชื้อเอดส์: รายงานผู้ป่วย 1 ราย

บทคัดย่อ
Cryptococcosis ในไขกระดูกเป็นอาการแสดงที่พบได้น้อยอย่างหนึ่งของโรค disseminated cryptococcosis เกิดจากการติดเชื้อรา Cryptococcus neoformans เกือบทั้งหมดของผู้ป่วยพบในผู้ติดเชื้อเอชไอวี การศึกษานี้เป็นรายงานผู้ป่วยชายไทยอายุ 39 ปี มาตรวจด้วยอาการปวดศีรษะและมีไข้ ภายใน 1 สัปดาห์ อาการปวดเป็นมากขึ้นเรื่อย ๆ การตรวจเลือดพบว่าให้ผลบวกกับ HIV antigen/antibody ร่วมกับภาวะโลหิตจางและเม็ดเลือดขาวต่ำตรวจนับ CD4 ได้เพียง 5/mm3 เท่านั้น ตรวจน้ำหล่อสมองและไขสันหลังพบ budding encapsulated yeasts ตรวจชิ้นเนื้อไขกระดูกก็พบ cryptococcosis เช่นกัน ผลเพาะเชื้อจากเลือดและน้ำหล่อสมองและไขสันหลังได้เชื้อ C. neoformans เหมือนกัน จึงให้การวินิจฉัยว่าเป็นโรค disseminated cryptococcosis ในผู้ป่วยติดเชื้อเอชไอวีโดยมีภาวะเยื่อหุ้มสมองอักเสบจากเชื้อ C. neoformans และยังพบ cryptococcosis ในชิ้นเนื้อไขกระดูกด้วยหลังจากให้การรักษาด้วย amphotericin B ร่วมกับการเจาะหลังบ่อย ๆ ต่อด้วยการให้รับประทาน fluconazole ผู้ป่วยตอบสนองดีมาก แล้วจึงให้การรักษาด้วยยา ARV ในภายหลังภาวะโลหิตจางชนิด normochromic normocytic และภาวะเม็ดเลือดขาวต่ำในผู้ป่วยรายนี้ เชื่อว่าน่าจะเกี่ยวข้องกับการพบเชื้อราในไขกระดูกโดยตรง เพราะเพียงแต่ให้การรักษาเชื้อรา อาการโลหิตจาง และเม็ดเลือดขาวต่ำก็ดีขึ้น
