การศึกษาย้อนหลัง 5 ปี ในผู้ป่วยโรคบาดทะยัก ของโรงพยาบาลมหาราชนครราชสีมา

บทคัดย่อ
วัตถุประสงค์: เพื่อศึกษาลักษณะทางคลินิก การรักษา การเกิดภาวะแทรกซ้อนและอัตราการเสียชีวิตของผู้ป่วยโรคบาดทะยักในโรงพยาบาลมหาราชนครราชสีมา ผู้ป่วยและวิธีการ: รวบรวมข้อมูลพื้นฐานและลักษณะทางคลินิกของผู้ป่วยบาดทะยักจากเวชระเบียน ในโรงพยาบาลมหาราชนครราชสีมา ตั้งแต่เดือน มกราคม พ.ศ. 2549 ถึงเดือน ธันวาคม พ.ศ. 2553 นำเสนอข้อมูลในรูปแบบพรรณนาและวิเคราะห์ข้อมูลทางสถิติ ผลการศึกษา: มีผู้ป่วยจำนวน 51 ราย เป็นชายร้อยละ 74.5 อายุเฉลี่ย 63.6 ± 12.9 ปี (พิสัย 36-90 ปี) ประมาณร้อยละ60 ของผู้ป่วยมีอายุมากว่า 60 ปี ผู้ป่วยร้อยละ 60.8 ประกอบอาชีพเกษตรกรรม ประวัติบาดแผลก่อนเกิดอาการพบร้อยละ 58.8 ไม่มีผู้ป่วยรวยใดได้รับวัคซีนป้องกันโรคหลังจากมีบาดแผลและมีผู้ป่วยเสียชีวิต 1 ราย ไม่พบความสัมพันธ์ระหว่าง อายุ เพศ ระยะเวลาฟักตัว ระยะเวลาก่อโรค กับระดับความรุนแรงของโรคแทรกซ้อนแต่พบว่าผู้ป่วยที่มีความรุนแรงของโรคสูงมีอัตราการเกิดภาวะแทรกซ้อนและมีระยะเวลารักษาตัวในโรงพยาบาลนานกว่ากลุ่มผู้ป่วยที่มีความรุนแรงของโรคต่ำ สรุป: ลักษณะทางคลินิกในผู้ป่วยโรคบาดทะยักขอและเกิดภาวะแทรกซ้อน ผู้ป่วยที่มีอาการของโรคที่รุนแรงมีความเสี่ยงต่อการเกิดภาวะแทรกซ้อนและมีระยะเวลารักษาตัวในโรงพยาบาลนานกว่าอย่างมีนัยสำคัญ การเจาะคอตั้งแต่ระยะแรกการได้รับการดูแลในหอผู้ป่วยวิกฤตและการรักษาภาวะแทรกซ้อนที่ดีเป็นปัจจัยที่ช่วยลดอัตราการเสียชีวิต
