บทคัดย่อ
รายงานผู้ป่วยเด็กหญิงอายุ 18 วัน มาโรงพยาบาลด้วยก้อนเลือดที่หลังมือ และจ้ำเขียวที่แขนและขา ตรวจการแข็งตัวของเลือดพบว่ามีค่า Prothrombin time 54.7 วินาที (10.0-15.1 วินาที) INR 4.27 ซึ่งยาวกว่าปกติ, activated partial thromboplastin time 49.3 วินาที (29.5-45.5 วินาที), Thrombin time 18.2 วินาที (14-21.0 วินาที) จากการตรวจอัลตราซาวด์และเอกซเรย์คอมพิวเตอร์สมองพบมีเลือดออกในสมองร่วมด้วย ผลการตรวจระดับแฟคเตอร์ VII เท่ากับ 2% ซึ่งยืนยันเป็น congenital factor VII deficiency หลังได้การวินิจฉัย ผู้ป่วยได้รับแฟคเตอร์ VII ทดแทนในรูปพลาสมาสดแช่แข็งอาการดีขึ้น และให้พลาสมาสดแช่แข็ง สัปดาห์ละ 3 ครั้ง เป็นการป้องกันเลือดออกซ้ำ ได้ติดตามพัฒนาการอยู่ในเกณฑ์ปกติ
