ประสบการณ์การรักษา Acute Stroke ด้วย rtPA ในโรงพยาบาลมหาราชนครราชสีมา

บทคัดย่อ
วัตถุประสงค์: เพื่อรายงานประสบการณ์การรักษา acute stroke ด้วย recombinant tissue plasminogen activator (rtPA) ใน Ischemic Stroke Fast Tract Program (ISFTP) ของโรงพยาบาลมหาราชนครราชสีมา ผู้ป่วยและวิธีการศึกษา: ผู้ป่วยโรคสมองขาดเลือดเฉียบพลันภายใน 3 ชั่วโมง ในโรงพยาบาลมหาราชนครราชสีมา ที่เข้าเงื่อนไขของ ISFTP ซึ่งได้รับการรักษาด้วยยาละลายลิ่มเลือดทางหลอดเลือดดำr tPA ระหว่าง 1 กรกฎาคม-30 พฤศจิกายน 2551 ได้รับการตรวจร่างกายทางระบบประสาทวิทยา และตรวจร่างกายทั่วไปตาม National Institute Stroke Scale, modified Rakin scale และ Barthel index รวมทั้งได้ตรวจเอกซเรย์คอมพิวเตอร์สมองก่อนให้ยาและหลังให้ยาแล้ว 24 ชั่วโมง ผลการศึกษา: ผู้ป่วย 5 รายที่เข้าเงื่อนไขดังกล่าว โดยทุกคนมีรอยโรคที่m iddle cerebral artery และมีการเต้นของหัวใจผิดปกติชนิด atrial fibrilation หลังให้ยาพบว่าผู้ป่วย 3 ราย ร่างกายฟื้น สภาพเป็นปกติ อีก 2 ราย มีอาการดีขึ้น แต่มีเลือดตกในเนื้อสมองเป็นภาวะแทรกซ้อน สรุป: การรักษาด้วย rtPA ใน Ischemic Stroke Fast Tract Program ของโรงพยาบาลมหาราชนครราชสีมา ได้ผลเป็นที่น่าพอใจและควรคงโครงการนี้ต่อไป
