การพัฒนารูปแบบใหม่ของคลินิกบริการผู้ป่วยเบาหวานและประสิทธิผลการให้บริการที่สถานีอนามัย ตำบลหนองหัวแรต อำเภอหนองบุญมาก จังหวัดนครราชสีมา

บทคัดย่อ

          The objective of this study was to test a new model of diabetes service and its effectiveness at Nonghuarat Health Center, Nongboonmark, Nakhon Ratchasima. Action research, before-after experiment, was deployed between April and December 2004. The participants were 42 diabetes type 2. They were interviewed and examined by the researcher and health center team. Data included demographic characteristics, health behavior, patient satisfaction, HbA1c blood test, and body mass index (BMI). Descriptive statistics including frequency, percentage, means, and standard deviation, were used. Paired t-test was done in order to test statistical significance. The results revealed that there were improvements in many aspects such as means of knowledge about disease, food consumption, exercise, and follow up appointment. HbA1c and BMI decreased with statistical significance (P = 0.000). At the health center, the new diabetic care model covered physical, psychological, socioeconomic, continuity of care, integrated care and health promotion and prevention. Patients were classified by their blood sugar control. Activities were self help group and home visit. After experiment, increase of patient satisfaction level was found. In conclusion, the new diabetic health center care model met targeted objectives, especially knowledge of disease and health behaviors consisting of food consumption, exercises, and follow-up appointments. It also demonstrated the achievement of HbA1c, BMI and patient satisfaction goals.
