การระงับปวดหลังผ่าตัดกระดูกโดยวิธีฉีดมอร์ฟีนเข้าช่องไขสันหลัง

บทคัดย่อ: โรงพยาบาลมหาราชนครราชสีมามีการให้บริการทางด้านศัลยกรรมกระดูกและข้อเป็นจำนวนมากในแต่ละปี ปัญหาความเจ็บปวดจากการผ่าตัดกระดูกนับเป็นปัญหาสำคัญที่มักถูกละเลย ผู้วิจัยจึงได้ทำการศึกษาเทคนิคการให้ยาระงับความปวดโดยวิธีฉีดมอร์ฟีนเข้าช่องไขสันหลัง ร่วมกับยาชาเพื่อหวังผลในการบรรเทาอาการปวดหลังผ่า ตัด และลดภาวะแทรกซ้อนที่เกิดขึ้นเนื่องจากความเจ็บปวด รวมถึงศึกษาอุบัติการณ์ของภาวะแทรกซ้อนจากเทคนิคดังกล่าว โดยศึกษาในผู้ป่วยที่มารับการผ่าตัดกระดูกบริเวณขาจำนวน 124 ราย พบภาวะแทรกซ้อน ได้แก่ อาการคัน ร้อยละ 60.5 ปัสสาวะคั่ง ร้อยละ 35.8 คลื่นไส้อาเจียนร้อยละ 26.6 ง่วงซึมร้อยละ 1.6 และไม่พบการกดการหายใจ ส่วนฤทธิ์ในการระงับปวดพบว่าผู้ป่วยไม่ต้องใช้ยาฉีดแก้ปวดภายใน 24 ชั่วโมงแรกถึงร้อยละ 78.3 ในผู้ป่วยที่ต้องการยาฉีดแก้ปวดก็เริ่มขอยาฉีดหลังการทำเทคนิคนี้โดยเฉลี่ย 9.7 ชั่วโมง
