การรักษาบาดแผลทะลุที่ลำไส้ใหญ่โดยวิธีเย็บปิดธรรมดา

บทคัดย่อ
ผู้ป่วยบาดเจ็บที่ลำไส้ใหญ่ซึ่งได้รับการรักษาที่โรงพยาบาลมหาราชนครราชสีมา ตั้งแต่วันที่ 1 มกราคม 2528 ถึง31 ธันวาคม 2539 จำนวน 176 ราย ได้ถูกนำมาศึกษาผลของการรักษาโดยการทำ primary repair พบว่ามีการรักษาโดยการทำ primary repair 118 ราย (ร้อยละ 67) โดย 18 ราย(ร้อยละ15) เกิดภาวะแทรกซ้อนจากการติดเชื้อหลังผ่าตัด และ 5 ราย (ร้อยละ 4) เสียชีวิตจากภาวะแทรกช้อน ปัจจัยเสี่ยงที่มีผลต่อภาวะแทรกช้อนจากการติดเชื้ออย่างชัดเจนคือ บาดเจ็บร่วมของอวัยวะในช่องท้องเกิน 2 อวัยวะ (p < 0.01) และบาดแผลที่ลำไส้ใหญ่มีความรุนแรงจนต้องมีการตัดต่อ (p < 0.001) ส่วนสาเหตุของการบาดเจ็บ ตำแหน่งของการบาดเจ็บ และเวลาจากที่เกิดการบาดเจ็บจนถึงเวลาที่ผ่าตัด ไม่มีผลกระทบต่ออัตราตายและภาวะแทรกซ้อนของการทำ primary repair
